1. The subject of the contract is the purchase of a computer tomograph with adaptation and adaptation of rooms in the John Paul II Podkarpackie Provincial Hospital in Krosno

2. The scope of the contract, boundary parameters, system functionality and conditions related to delivery are set out in Annex 2a, 2b, 2c to the SWZ.

1. The subject of the contract is the purchase of a computer tomograph with adaptation and adaptation of rooms in the John Paul II Podkarpackie Provincial Hospital in Krosno.

2. The scope of the contract, boundary parameters, system functionality and conditions related to delivery are set out in Annex 2a, 2b, 2c to the SWZ.

3. The scope of the subject of the order includes:

1) delivery of a computer tomograph in accordance with the description of the subject of the contract, which constitutes Annex 2a to the SWZ

2) execution of works related to the adaptation and adaptation of rooms in building B of the John Paul II Podkarpackie Provincial Hospital in Krosno at 57 Korczyńska Street for the needs of the computed tomography laboratory together with a description room and delivery of furniture and equipment in accordance with Annexes 2b and 2c to the SWZ, design documentation, building permit and tender offer of the Contractor of ..........., which form an integral part of the contract,

3) Preparation of the design of fixed shields (along with calculations) for the supplied computer tomograph, agreement on the project with the Podkarpackie State Provincial Sanitary Inspector, execution of shields based on the design of fixed shields. After completion of the works, the Contractor will perform tests on the effectiveness of shields in the CT room by an accredited research unit with the submission of the report to the Contracting Authority and will obtain permission to start and use the CT apparatus and the CT laboratory for medical diagnostics.

4) obtaining, in the name and on behalf of the Ordering Party, on the basis of the granted power of attorney, all necessary decisions, conditions and arrangements and making all necessary notifications in the name and on behalf of the Ordering Party, in particular:

- notifications on the completion of construction to the District Building Supervision Inspector (hereinafter: PINB), the State District Sanitary Inspector (hereinafter: Sanepid) and to the State Fire Service (hereinafter: Fire Service) and obtaining a positive opinion of these institutions as to the commencement of use of the reconstructed premises or submitting a statement that these authorities have not objected to the commencement of use within the deadlines specified in the law - if required,

- permits for the launch and use of the CT apparatus and the CT laboratory for medical diagnostics issued by the Podkarpackie State Provincial Sanitary Inspectorate.

- as-built inventory.

5) Conducting training:

-application for technicians and doctors at the headquarters of the Ordering Party

- technical personnel in the field of basic operation, operation and maintenance
