                        Description:
                        
                        Delivery of disposable medical devices for the needs of SPZOZ in Myszków
The subject of the contract are successive deliveries of disposable medical devices for the needs of the Independent Public Health Care Team in Myszków. The order is divided into 43 packages (parts). Agreements for individual packages will be concluded after the completion of these proceedings for a period of 12 months. A detailed description of the subject of the contract and the conditions for delivery shall be specified by:
- Price form – Annex 1.1 to SWZ,
- draft provisions of the agreement – Annex 2 to the SWZ.
All requirements set out in the documents indicated above constitute minimum requirements and their fulfilment is obligatory. Failure to comply with the minimum requirements will result in the rejection of the offer as incompatible with the terms of the contract pursuant to Article 226 (1) (5) of the Law Pzp.
