The subject matter of this public procurement procedure is:

1. delivery of 7 packages of intraocular lenses with the simultaneous creation of a deposit warehouse for the following packages: No. 1, No. 3, No. 5, No. 7 for the needs of the Ophthalmology Department of the Provincial Hospital in Elbląg. A detailed description, requirements and quantities are specified in Appendices No. 1 to No. 7 to the Terms of Reference.

2. creation by the Contractor at the Contracting Authority's registered office of a free-of-charge deposit warehouse within the scope of package No. 1, No. 3, No. 5, No. 7 on the terms and conditions that have been regulated by the deposit agreement (the template is attached as Appendix No. 15 to the Terms of Reference), constituting an integral part of the sales agreement, within a period of no more than 8 working days.
