The procedure is conducted in an open tender procedure pursuant to Article 132 of the Public Procurement Law.The subject of the contract is the supply of medical devices for bariatric procedures according to the following parts: Task 1 - Task 7. A detailed description of the subject matter of the contract for the above-mentioned part has been shown and described in the "Price Form" (Appendix 2) and in the Model Agreement I (Appendix 3), in Model Agreement II (Appendix 3a) and Model Agreement III (Appendix 3b), which are an integral part of the Terms of Reference. The contractual terms and conditions for the performance of the public contract in question contain model contracts, which are an integral part of the Terms of Reference. The estimated value of the subject of the contract without the option right is PLN 2,723,921.00. Section II.1.5. an estimated total value including the right of option.

The subject of the contract is the supply of medical devices for bariatric procedures. The above-mentioned part of the subject matter of the contract has been shown and described in the "Price Form" constituting an integral part of the Terms of Reference - Part No. 1 (4 items). In the case of Task 1, the Contractor shall provide the Contracting Authority with a universal laparoscopic stapler specified in this part of the subject matter of the contract. This will be made available through the lease of the above-mentioned universal laparoscopic stapler by the Contractor to the Contracting Authority for the duration of the contract. The Contractor is obliged to lease a complete device, i.e. one that allows it to be used by the Ordering Party without the need to purchase additional accessories. The terms of the above-mentioned provision are described in the model agreement, which is an integral part of the Terms of Reference. For Task 1, the Contractor, whose offer has been selected by the Contracting Authority as the most advantageous before concluding the post-tender agreement, will evaluate the offered universal laparoscopic stapler in accordance with the "List of leased equipment", which will constitute Appendix No. 2 to the contract.

The subject of the contract is the supply of medical devices for bariatric procedures. The above-mentioned part of the subject matter of the contract has been shown and described in the "Price Form" constituting an integral part of the Terms of Reference - Lot No. 6 (2 items),

The subject of the contract is the supply of medical devices for bariatric procedures. The above-mentioned part of the subject matter of the contract has been shown and described in the "Price Form" constituting an integral part of the Terms of Reference - Lot No. 7 (3 items). The Contractor shall deposit at the Contracting Authority's registered office the selected products indicated in the "Price Form" in the form of a deposit. The rules of deposit of selected products are described in the model agreement, which is an integral part of the Terms of Reference
