Delivery of orthopedic implants to SPZZOZ in Wyszków within 12 months

The subject of the contract is "Delivery of orthopedic implants to SPZZOZ in Wyszków

within a period of 12 months" - Packages from No. 1 to No. 9.

The description of the subject of the contract is included in Appendix No. 2 OPZ – Detailed price offer and Appendix No. 5 Model contract.

Payment deadline for the implementation of the subject of the order: 60 days from the date of delivery to the Ordering Party of a correctly issued invoice.

The delivered goods must meet the quality conditions specified in the specifications of the terms of the contract and in the Contractor's tender offer and must have appropriate certificates, attestations and approvals for marketing and use in the health service in accordance with applicable law, including for products classified as medical devices within the meaning of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2021, item 1565, as amended),

The validity period of the subject of the contract must be a minimum of 12 months from the date of delivery.
