The subject of the order includes the collection and development (recovery or disposal), indicated in the description of the order, municipal waste from properties located within the municipality of Milicz, the operation of the Selective Collection of Municipal Waste (PSZOK) and the collection of waste from the PSZOK in Stawcu 40b, 56-300 Milicz along with transportation and development of collected waste. Contractors of parts 1 and parts 4, 7, 8, 9, 10 are required to provide the service in a way that ensures that levels of recycling and preparation for the reuse of municipal waste and the level of limitation of mass of biodegradable waste transferred to storage in accordance with the current provisions of the Act of 13 September 1996 on the maintenance of cleanliness and order in the municipalities and the current implementing acts to the above Act, as well as in the records of local law. A detailed description of the item of the order is specified in the SWZ for the service in question.
Part 1 - collection and management of waste received “at the source of municipal waste”Part 1: collection and management of waste received 'at source of municipal waste' with codes: 20 03 01, 15 01 02, 20 01 39, 15 01 07, 20 01 02, 15 01 02, 15 01 04, 20 01 40, 15 01 01 01, 20 01 01 01, 20 02 01 08.
- The provision of services for the collection of municipal waste generated in the territory of the municipality of Milicz, includes the obligation of Contractors to:
1) Receiving from owners of domiciled property
a.municipal waste,
b.mixed municipal waste,
c.biowaste, collected in a selective manner, as set out in the Rules of Procedure.
d.glass waste collected selectively as set out in the Regulations - in any quantity,
e.paper waste collected selectively as set out in the Rules of Procedure.
f.waste from plastics, metals and multi-plant packaging collected selectively as set out in the Regulations.
2) Receipt from owners of uninhabited property (including land parcel gardens) on which waste is created, municipal waste including:
a.mixed municipal waste — in the quantity resulting from the number of containers declared by the property owner, taking into account the weekly waste generation standards set out in the Regulations,
b.biowaste, collected selectively as set out in the Regulations,
c.glass waste collected selectively as set out in the Regulations,
d.paper waste collected selectively as set out in the Rules of Procedure,
e.waste of plastic, metals and multi-plant packaging collected selectively as set out in the Regulations.
3) Receipt from property owners built up cottages or from other properties used for recreational and leisure purposes used only for part of the year (according to the list of real estate — Annex 13 to SWZ), including:
a.municipal waste mixed,
b.biowaste, collected selectively as set out in the Regulations,
c.glass waste collected selectively as set out in the Regulations,
d.paper waste collected selectively as set out in the Rules of Procedure,
e.waste of plastic, metals and multi-plant packaging collected selectively as set out in the Regulations.
4) Systematic, timetable, reception of mixed municipal waste.
5) Systematic, timetable, receipt of municipal waste collected selectively. The Contracting Authority indicates that the selective collection “at source” will be broken down into the following fractions:
a.paper,
b.plastics, multi-label packaging, metals,
c.glass,
d.biowaste,
6) Receiving municipal waste from property owners inhabited with a minimum frequency of:
a.mixed municipal waste — 1 time every two weeks,
b.plastics, multi-cartridges, metals — 1 time a month,
c.glass — 1 time a month,
d.paper — 1 time a month,
e.biowaste:
(a) from 1 October to 31 March — 1 time per month
(b) from 1 April to 30 September — every 2 weeks.
7) Receiving municipal waste from property owners inhabited by multi-family buildings with a minimum frequency (with section 7a):
a.mixed municipal waste — 1 time a week,
b.plastics, multi-cartridges, metals — 2 times a month,
c.paper — 2 times a month,
d.glass — 2 times a month,
e.biowaste:
(a) between 1 October and 31 March — 2 times a month
(b) from 1 April to 30 September — 1 weekly.
7a.Reception of mixed municipal waste from the site of the Housing Cooperative “Barycz” is to be carried out at a frequency of 2 x per week.
The collection of segregated fractions referred to in point 7 (b) to (e) from the site of the Barycz Housing Cooperative is to be carried out at a frequency of 1 x per week.

Part 10: collection and development from the Selective Collection Point of Municipal Waste (PSZOK) in Stawec fraction of plastic waste, metals, multi-plant packagingPart 10: Receipt and zoning from the Selective Collection Point of Municipal Waste (PSZOK) in the Waste Faction Stawec with codes 20 01 39, 15 01 02, 15 01 05, 15 01 04, 20 01 40.
1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:
(...) (b) metals with codes: 15 01 04 and 20 01 40, (i) multi-plant packaging code 15 01 05, l) plastic waste codes: 20 01 39, 15 01 02,
2) The Contracting Authority requires the receipt and transportation and ongoing development of the waste fraction from the PSZOK.
3) The Contractor receives waste from the Selective Collective Waste Collection Point, in accordance with the scope of the contract concluded with him,
4) Since it is the Contractor who receives waste from the PSZOK, the loading and transportation and landscaping is on the Contractor's side, so it is the Contractor who decides how to collect the waste (containers/bags).
5) It is forbidden to mix individual fractions of waste with each other.
6) It is forbidden to mix municipal waste received from the area of the municipality of Milicz with waste collected from the territory of another municipality.
7) Reception of waste from the PSZOK will occur during the days and hours of its opening.
8) The Contractor should make a local vision before submitting an offer in order to familiarize himself with the local conditions and specifics of the place where the PSZOK is located.
9) Each time before starting on a given day of receipt of waste from the PSZOK, the Contractor is obliged to weigh the empty vehicle (perform friction). This action should be performed on the weight in the PSZOK. Failure to comply with this obligation will result in the absence of the possibility of recognizing this waste as originating in the Milicz Municipality and the Ordering Party will not pay for its collection and development.
10) The Contracting Authority stipulates that all municipal waste collected or collected under the subject of this order from the Milicz Municipality and leaving for a municipal installation or other authorised installation shall be weighed on a legalised weight fitted in the PSZOK, and this measurement constitutes a basis for accruing by the Contracting Authority to the Contractor,
11) The Contracting Authority requires the ongoing management and receipt of waste from the Selective Waste Collection Point (PSZOK) located in Stawec.
12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act;

Part 11: collection and development from the Selective Collection of Municipal Waste (PSZOK) in Stawec of selected waste fractionsPart 11: collection and management of selected waste fractions with codes 20 01 33 *, 20 01 80, 20 01 29*, 20 01 30, 20 01 13 *, 20 01 33 *, 20 01 34, 20 01 80, 20 01 29*, 20 01 30, 20 01 13 *, 20 01 14*, 20 01 15*, 20 01 17*, 20 01 19*, 20 01 10, 20 01 11, and hazardous waste secreted from the stream of municipal waste, waste ineligible for household medical waste due to intake of medicinal products in the form of injections and monitoring of blood substances, in particular needles and syringes.
1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:
(...) e) batteries and batteries with codes: 20 01 33*, 20 01 34,
g) expired drugs and chemicals with codes: 20 01 80, 20 01 29*, 20 01 30, 20 01 13*, 20 01 14*, 20 01 15*, 20 01 17*, 20 01 19*,
(j) textiles and clothing with codes: 20 01 10, 20 01 11;
(o) waste ineligible for household medical waste due to intake of medicinal products by injection and monitoring of blood levels, in particular needles and syringes.
2) The Contracting Authority requires the receipt and transportation and ongoing development of the waste fraction from the PSZOK.
3) The Contractor receives waste from the Selective Collective Waste Collection Point, in accordance with the scope of the contract concluded with him,
4) Since it is the Contractor who receives waste from the PSZOK, the loading and transportation and landscaping is on the Contractor's side, so it is the Contractor who decides how to collect the waste (containers/bags).

12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act;

Part 2 - Running the Selective Collection of Municipal Waste (PSZOK) in StawcuOrder fulfillment conditions for Part 2:
In the framework of the implementation of the work related to the conduct (organizing) of the Selective Collective of Municipal Waste:
1) Deleted;
2) The Contracting Authority shall provide at the disposal of the Contractor a part of the site with an area of approximately 0,22 hectares in Stawiec on plot no 34/4 AM 1, which is to be used in a minimum of 75% of the area to organize the Selective Waste Collections Point (PSZOK);
3) The Contracting Authority shall indicate the minimum requirements for the PSZOK:
(a) The Contractor shall equip the PSZOK with a legalised weight and data recorder on the owner of the property and the municipal waste donated by him, ensuring continuity of the weighting of vehicles and waste after completion of the existing contract by 1.01.2022 at the latest,
(b) PSZOK will provide services from Monday to Friday from 6:00 to 18:00,
(c) The Contractor shall inform all owners of the property covered by the waste collection of the PSZOK's operating rules, its operation and its operating hours, within 14 days from the date of commissioning the PSZOK, in particular by:
- posting information on the website of the Contractor and the Contracting Authority,
- in the local press;
4. PSZOK accepts the following waste in accordance with local law:
(a) furniture and other large-sized waste code 20 03 07,
(b) metals with codes: 15 01 04 and 20 01 40,
(c) consumed tyres of code 16 01 03 in quantities up to 4 units per year per household concerned,
(d) construction and demolition waste with codes: 17 01 01 01, 17 01 02, 17 01 07, 17 09 04 in a total maximum quantity not exceeding 500 kg per half year,
e) batteries and batteries with codes: 20 01 33*, 20 01 34,
(f) consumed electrical and electronic equipment with codes: 20 01 35*, 20 01 36, 20 01 23*, 20 01 21*.
g) expired drugs and chemicals with codes: 20 01 80, 20 01 29*, 20 01 30, 20 01 13*, 20 01 14*, 20 01 15*, 20 01 17*, 20 01 19*,
(h) paper with codes: 15 01 01 01, 20 01 01,
(i) multi-aerial packaging code 15 01 05,
(j) textiles and clothing codes: 20 01 10, 20 01 11,
(k) biowaste codes: 20 01 08, 20 02 01,
(l) plastic waste codes: 20 01 39, 15 01 02,
m) glass waste with codes: 15 01 07, 20 01 02,
(n) hazardous waste secreted from the stream of municipal waste,
(o) waste ineligible for household medical waste due to intake of medicinal products by injection and monitoring of blood levels, in particular needles and syringes.
5) The Contractor shall equip the PSZOK, at the latest 3 days prior to the start of the contract, with containers intended for the collection of waste referred to in point 4).
6. The equipment of the PSZOK point must be:
(a) adequate for the type of waste received,
(b) complying with all technical and legal requirements relating to the waste type concerned,
(c) for ease and comfort when service/use by residents.
7) The Contractor is obliged to weigh all municipal waste harved/collected under Part 1 of the object of the contract from the territory of the municipality of Milicz both entering the weight in the PSZOK and leaving for the municipal installation or other authorised installation.
7a) The Contractor shall weigh the empty vehicles of the Contractor Part 1 each time before starting on the day of the waste collection.
7b) The Contractor shall weigh empty vehicles to contractors Parts 3, 4, 5, 6, 7, 8, 9, 10, 11 each time when receiving waste from PSZOK.
8) The Contractor is obliged to provide on the territory of the PSZOK (but not under the PSZOK) the possibility of receiving and landscaping from the owners of the property of the municipality Milicz (“at source” or after the delivery of waste to the Contractor) of “problem” waste, such as: construction polystyrene, wool Mineral, papa — at the expense of the property owner, who intends to get rid of these waste. In this case, the financial settlement is only between the Contractor and the supplier of the waste.

1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:

5) It is forbidden to mix individual fractions of waste with each other.

7) Reception of waste from the PSZOK will occur during the days and hours of its opening.

10) The Contracting Authority stipulates that all municipal waste collected or collected under the subject of this order from the Milicz Municipality and leaving for a municipal installation or other authorised installation shall be weighed on a legalised weight fitted in the PSZOK, and this measurement constitutes a basis for accruing by the Contracting Authority to the Contractor,
11) The Contracting Authority requires the ongoing management and receipt of waste from the Selective Waste Collection Point (PSZOK) located in Stawec.
12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act.
Part 4 - collection and development from the Selective Collection Point of Municipal Waste (PSZOK) in Stawec fraction of post-assembly wastePart 4 - collection and development from the Selective Collection Point of Municipal Waste (PSZOK) in Stawec fraction of post-assembly waste.
1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:

1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:
(...) (c) consumed tyres of code 16 01 03 in quantities up to 4 units per year per household in question ( ... ),
2) The Contracting Authority requires the receipt and transportation and ongoing development of the waste fraction from the PSZOK.
3) The Contractor receives waste from the Selective Collective Waste Collection Point, in accordance with the scope of the contract concluded with him,
4) Since it is the Contractor who receives waste from the PSZOK, the loading and transportation and landscaping is on the Contractor's side, so it is the Contractor who decides how to collect the waste (containers/bags).

10) The Contracting Authority stipulates that all municipal waste collected or collected under the subject of this order from the Milicz Municipality and leaving for a municipal installation or other authorised installation shall be weighed on a legalised weight fitted in the PSZOK, and this measurement constitutes a basis for accruing by the Contracting Authority to the Contractor,
11) The Contracting Authority requires the ongoing management and receipt of waste from the Selective Waste Collection Point (PSZOK) located in Stawec.
12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act.

1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:

4) Since it is the Contractor who receives waste from the PSZOK, the loading and transportation and landscaping is on the Contractor's side, so it is the Contractor who decides how to collect the waste (containers/bags).
5) It is forbidden to mix individual fractions of waste with each other.
6) It is forbidden to mix municipal waste received from the area of the municipality of Milicz with waste collected from the territory of another municipality.

11) The Contracting Authority requires the ongoing management and receipt of waste from the Selective Waste Collection Point (PSZOK) located in Stawec.
12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act;

Part 8: collection and development from the Selective Collection Point of Municipal Waste (PSZOK) at the Glass-Waste FractionPart 8: Receipt and landscaping from the Selective Collection Point of Municipal Waste (PSZOK) in Stawiec of the glass-waste fraction.
1) The Contractor shall receive and transport and use all of the following or selected waste fractions from the PSZOK:
(...) m) glass waste with codes: 15 01 07, 20 01 02,
2) The Contracting Authority requires the receipt and transportation and ongoing development of the waste fraction from the PSZOK.
3) The Contractor receives waste from the Selective Collective Waste Collection Point, in accordance with the scope of the contract concluded with him,
4) Since it is the Contractor who receives waste from the PSZOK, the loading and transportation and landscaping is on the Contractor's side, so it is the Contractor who decides how to collect the waste (containers/bags).
5) It is forbidden to mix individual fractions of waste with each other.
6) It is forbidden to mix municipal waste received from the area of the municipality of Milicz with waste collected from the territory of another municipality.
7) Reception of waste from the PSZOK will occur during the days and hours of its opening.
8) The Contractor should make a local vision before submitting an offer in order to familiarize himself with the local conditions and specifics of the place where the PSZOK is located.
9) Each time before starting on a given day of receipt of waste from the PSZOK, the Contractor is obliged to weigh the empty vehicle (perform friction). This action should be performed on the weight in the PSZOK. Failure to comply with this obligation will result in the absence of the possibility of recognizing this waste as originating in the Milicz Municipality and the Ordering Party will not pay for its collection and development.
10) The Contracting Authority stipulates that all municipal waste collected or collected under the subject of this order from the Milicz Municipality and leaving for a municipal installation or other authorised installation shall be weighed on a legalised weight fitted in the PSZOK, and this measurement constitutes a basis for accruing by the Contracting Authority to the Contractor,
11) The Contracting Authority requires the ongoing management and receipt of waste from the Selective Waste Collection Point (PSZOK) located in Stawec.
12) In the field of waste management, the Contractor is obliged to transfer selectively collected municipal waste from PSZOK to waste recovery and disposal installations, in accordance with the hierarchy of waste handling referred to in the December 14, 2012 Waste Act;
