Technical Assistance and Maintenance Service of the Integrated Information System supporting the work of UMT departments - Otago system

1. The Contracting Authority entrusts the Contractor, and the Contractor accepts the performance of the service of Servicing the Otago System, consisting of Subsystems. The list of Subsystems can be found in Appendix 1 to the model agreement.

2. A detailed description of the subject matter of the contract is included in the model contract attached to the Terms of Reference.

3. The Contracting Authority requires the Contractor or subcontractor to employ on the basis of an employment contract, throughout the entire period of performance of the subject of the contract, persons performing the following activities during the performance of the subject of the contract: providing telephone consultations on the functioning of the Otago System Subsystems and its operation as part of the first line of support, which is obliged to answer the phone between 8:00 a.m. and 4:00 p.m. on Working Days (the so-called helpdesk). Details of the requirements for employment on the basis of an employment relationship are included in the model agreement attached to the Terms of Reference

4. Completion date - from 01.01.2024 to 31.12.2024

5. Tender deposit in the amount of: PLN 10,000.00 (say: ten thousand zloty)

should be lodged before the deadline for submission of tenders. The validity period of the defect should be in accordance with the date of binding the tender.

Vadium can be brought in the following forms:

(a) money - to UMT account No. 62 1160 2202 0000 0003 3943 1400 annotated:

Bid bond proceeding No. 132/2023 - "Otago System".

The date of the deposit of the defect shall be accepted for the date of its impact on the Ordering Party's

(b) bank surety or surety of the cooperative savings and credit bank,

(c) bank guarantees,

(d) insurance guarantees,

e) sureties granted by entities referred to in Article 6b(5)(2) of the Act of

9 November 2000 on the establishment of the Polish Agency for Enterprise Development (i.e. Journal of Laws of 2022,

items 2080, 2185).

If the defect is brought in the form of a surety or guarantee, the contractor shall forward the original warranty or surety to the contracting authority in electronic form.

The provisions concerning the return of the tender deposit are specified in point 16 of the Terms of Reference.
