1. The subject of the order is:
1) cooperation (cooperation) with the medical staff of the Ordering Party in the field of patient care,
2) the provision of cleanliness and disinfection services in the facilities of the Contracting Authority,
3) the provision of cleanliness services on the external premises owned by the Hospital with the care and reclamation of green areas and the snow removal and maintenance of conditions for its safe use during the winter,
4) the provision of internal transport services within the facilities of the Ordering Authority and between the facilities of the Ordering Authority located in Legnica at 5J. Iwaszkiewicza Street,
5) running a deposit of clothing and belongings of patients and visitors to them.
2. Information clause regarding the processing of personal data - has been included in Chapter XXVI of the Terms of Reference.

2. The conditions and method of execution of the order are also described in:
1. Annexes to the SWZ,
2) the draft contract located in Chapter XX of SWZ.
3. The Contractor is obliged to provide materials and means necessary for the proper provision of services - the list of assortment and average annual consumption are indicated in Appendix B to the Terms of Reference
4. In order to perform the contract, the Contracting Authority shall hire the Contractor a room with a total area of 306,80 m² in accordance with the map constituting Annex A to the SWZ.
5. The Contractor is obliged to employ all persons who will participate in the performance of the service for the Contracting Authority on the basis of an employment contract – performing work, within the meaning of Article 22 § 1 of the Act of 26 June 1974 – Labour Code. It is the Contractor's responsibility to determine the number of working hours and other terms and conditions of individual employment contracts.
6. The description of the subject matter of the contract should be read together with any changes to the content of this Terms of Reference, e.g. as a result of replies to the Contractors' inquiries.
