1. The subject of the contract is: cleaning real estate administered by the ZGN Praga-Południe Capital City Warsaw, owned by the Capital City of Warsaw Warsaw together with running public toilets, located in:

1) AN-6 (part 1),

2) AN-7 (part 2),

3) AN-5, 6 and 7 (part 3): (cleaning the toilets).

2. Scope of work: specifies the Description of the subject of the contract (for parts 1, 2 and 3) together with the address list of the cleaning area (for parts 1 and 2), constituting Annex 3 to the ToR.

1. The subject of the contract is: cleaning real estate administered by the ZGN Praga-Południe Capital City Warsaw, owned by the Capital City of Warsaw Warsaw together with running public toilets located within the Property Administration No. 6.

2. Scope of work: specifies the Description of the object of the contract together with the address list of the cleaning area, constituting Annex 3 to the ToR, including

a) cleaning the hardened external surface, green areas in areas not served by caretakers (along with cleaning and cleaning of the sports facility at Majdańska St. - within the scope of part I (AN 6), carried out as part of a flat-rate monthly remuneration,

b) snow removal and de-icing of hardened external surfaces in areas not served by caretakers, is a periodic service - carried out in accordance with the frequency and scope specified by the Employer - without issuing an order,

c) performing the services referred to in point a) and b) (does not apply to the building at Majdańska St.) in areas served by caretakers, including inside buildings, in their absence - implemented only on commission issued 24 hours in advance,

3. Requirements for employment of persons pursuant to art. 29 paragraph 3a:

- The Employer requires that the Contractor or Subcontractor throughout the entire period of performance of the subject of the contract employ under a full-time employment contract at least: for part I - 4 persons providing work related to the performance of activities during the implementation of the subject of the contract (excluding staff managerial staff, persons with SEP qualifications and administrative employees). This requirement is also considered to be met in the case of employing a larger number of people employed part-time, if in total this employment is equivalent to the number of full-time 4 employees (for Part I) and this will not affect the correct performance of the contract,

- The above condition will be met by employing new employees under the employment contract or appointing employees already employed by the Contractor or Subcontractor to perform the contract,

- The scope of work to be carried out by persons employed under an employment contract is specified in the Description of the subject of the contract in point II.1 'sites' (for lot 1), and

In point II.4. "Opening, closing, and cleaning and cleaning of the sports facility at ul. Majdańska "(for part 1).

- The contractor shall, no later than 3 days after signing the contract, document the form of employment of the employees declared by him in the "List of persons who will participate in the performance of the contract", showing, for example, an employment contract, ZUS RCA printing, etc., which will clearly show the number of full posts. The documents referred to above should be anonymized in a manner ensuring protection of employees' personal data, in accordance with the provisions of the Act of August 29, 1997 on the protection of personal data (i.e. in particular without addresses, PESEL number of employees). The employee's first and last name are not anonymous. Information such as: date of the contract, type of employment contract and full-time employment should be identifiable,

- Sanctions for failure to comply with the requirements set out in paragraph 1 specifies the contract template constituting Annex 11 to the ToR (for part 1).

2. Scope of work: specifies the Description of the object of the contract together with the address list of the cleaning area, constituting Annex 3 to the ToR, including

a) cleaning of paved external surface, green areas in areas not served by caretakers, carried out as part of a monthly flat-rate remuneration,

b) snow removal and de-icing of hardened external surfaces in areas not served by caretakers, is a periodic service - carried out in accordance with the frequency and scope specified by the Employer - without issuing an order,

c) performing the services referred to in point a) and b) in areas served by caretakers, including inside buildings, in their absence - implemented only on the order issued 24 hours in advance,

d) cleaning the hardened external surface with a frequency of 2 to 8 times during the contract period (occasional cleaning), carried out each time on separate written orders (applies only to part II (AN-7).

3. Requirements for employment of persons pursuant to art. 29 paragraph 3a:

- The Employer requires that the Contractor or Subcontractor throughout the entire period of performance of the subject of the contract employ under a full-time employment contract at least: for part II - 4 people performing work related to the performance of activities during the implementation of the subject of the contract (excluding staff managerial staff, persons with SEP qualifications and administrative employees). This requirement is also considered to be met in the case of employing a larger number of people employed part-time, if in total this employment is equivalent to the number of full-time 4 employees (for part II) and this will not affect the correct performance of the contract,

- The above condition will be met by employing new employees under the employment contract or appointing employees already employed by the Contractor or Subcontractor to perform the contract,

- The scope of work to be carried out by persons employed under an employment contract is specified in the Description of the subject of the contract in point II.1 'sites' (for lot 2) ,,

- The contractor shall, no later than 3 days after signing the contract, document the form of employment of the employees declared by him in the "List of persons who will participate in the performance of the contract", showing, for example, an employment contract, ZUS RCA printing, etc., which will clearly show the number of full posts. The documents referred to above should be anonymized in a manner ensuring protection of employees' personal data, in accordance with the provisions of the Act of August 29, 1997 on the protection of personal data (i.e. in particular without addresses, PESEL number of employees). 1. The subject of the contract is the comprehensive provision of cleaning services for public toilets in the following locations: ul. Łukiska 29 (Znicza park) - AN6, 1 Szembeka Square - AN7 and ul. Grochowska 270 (Park Pak) - AN5.

Scope of work: specifies the Description of the object of the contract (for AN 5, AN 6 and AN7).

2. Requirements for employment of persons pursuant to art. 29 paragraph 3a:

- The Employer requires that the Contractor or Subcontractor throughout the entire period of performance of the subject of the contract employ under a full-time employment contract at least: for part III - 3 persons providing work related to the performance of activities during the implementation of the subject of the contract (excluding management staff , persons with SEP privileges and administrative employees). This requirement is also considered to be met if more people are employed on a part-time basis, provided that the total employment equals 3 full-time positions (for Part III) and this does not affect the correct performance of the contract,

- The above condition will be met by employing new employees under the employment contract or appointing employees already employed by the Contractor or Subcontractor to perform the contract,

- The scope of work to be carried out by persons employed under an employment contract is specified in the Description of the subject of the contract regarding the cleaning of restrooms (for lot 3),

- The contractor shall, no later than 3 days after signing the contract, document the form of employment of the employees declared by him in the "List of persons who will participate in the performance of the contract", showing, for example, an employment contract, ZUS RCA printing, etc., which will clearly show the number of full posts. The documents referred to above should be anonymized in a manner ensuring protection of employees' personal data, in accordance with the provisions of the Act of 29.8.1997 on the protection of personal data (i.e. in particular without addresses, PESEL number of employees). The employee's first and last name are not anonymous. Information such as: date of the contract, type of employment contract and full-time employment should be identifiable,

- Sanctions for failure to comply with the requirements set out in paragraph 1 specifies the contract template (for part 3) constituting Annex 11 to the ToR.
