1. The subject of the order is the delivery of teaching aids in the form of IT equipment and multimedia teaching aids in the project “Academy of Exemplary Education”.

2. The project “Academy of Exemplary Education” is implemented within the framework of the Regional Operational Programme of the Warmian-Masurian Voivodeship for 2014-2020, the priority axis of the 2nd Personnel for the economy, action 2.2. Increasing the quality of educational offerings focused on developing the competencies of key students, working 2.2.2 Increasing the quality of educational offerings focused on developing the competencies of key students — ZIT bis Elbląg projects.

3. The supply of IT equipment and multimedia teaching assistance is for the purposes of classes in the framework of the implementation of project No RPWM 02.02.02-28-0002/20. Purchase of equipment is necessary for the implementation of the project's assumptions, i.e. the enhancement of key competences and universal skills among Elbląan students elementary schools.

4. The scope of the order includes:

A. delivery of equipment in the form of IT equipment and multimedia teaching aids.

The detailed scope of the order shall include “DELIVERY VICE” constituting ANNEX 8 to SWZ.

B. assembly,

C. installation and commissioning,

D. Providing service during the warranty period for equipment that requires servicing.

3) a certificate of the competent head of the tax office confirming that the Contractor is not in arrears with the payment of taxes and fees, within the scope of Article 109(1)(1) of the PPL Act, issued not earlier than 3 months before its submission, and in the event of arrears with the payment of taxes or fees together with the certificate, the contracting authority requests the submission of documents confirming that before the deadline for submitting tenders, the Contractor has paid the due taxes or fees together with interest or fines, or has concluded a binding agreement on the repayment of those debts;

4) a certificate or other document of the competent field organizational unit of the Social Insurance Undertaking or the relevant regional branch or the relevant field facility of the Agricultural Social Insurance Service confirming that the Contractor is not in default on paying the premiums to social and health insurance, in the scope of Article 109 (1) (1) of the Pzp Act, issued no earlier than 3 months prior to its submission, and in the case of arrears of payment of social or health contributions, together with a certificate or other document, the Contracting Authority requests to submit documents proving that prior to the expiry of the deadline for the submission of tenders, the Contractor has made payments of the due social or health contributions, together with interest or fines, or has entered into a binding agreement on the repayment of those receivables;

5) a copy or information from the National Court Register or from the Central Registry and Business Activities Information, within the scope of Article 109 (1) (4) of the Pzp Act, drawn up no earlier than 3 months prior to its filing, if separate provisions require entry in the register or records;

6) the Contractor's statement on the validity of the information contained in the statement referred to in Article 125(1) of the PPL Act, regarding the grounds for exclusion from the procedure indicated by the Ordering Party (...)
