The subject of the contract is the purchase of a computer tomograph with equipment along with the adaptation of rooms in the Świętokrzyskie Psychiatry Center in Morawica at the price specified in the assortment and price form, which constitutes Appendix No. 2 to this specification. The scope of the contract, boundary parameters, system functionality and conditions related to delivery are specified in Appendix No. 2, 10 and 11 to SWZ.

The subject of the contract is the purchase of a computer tomograph with equipment along with the adaptation of rooms in the Świętokrzyskie Psychiatry Center in Morawica at the price specified in the assortment and price form, which constitutes Appendix No. 2 to this specification. The scope of the contract, boundary parameters, system functionality and conditions related to delivery are specified in Appendix No. 2, 10 and 11 to SWZ.

The scope of the contract includes:

a) adaptation and installation of the delivered subject of the contract in the computed tomography laboratory and its adaptation to the applicable requirements in this respect. Adaptation in accordance with the scope of adaptation and construction works, which was specified in Annex 10 to the SWZ,

b) purchase, delivery, unloading, bringing, assembly of the device and commissioning in the place indicated by the Ordering Party together with all components and accessories of the tomograph, its equipment, accessories, auxiliary and cooperating devices and software,

c) training in the operation of the system for doctors, technicians and other persons authorized to operate a computer tomograph at the headquarters of the Ordering Party within the time agreed with the Ordering Party after launching all elements of the subject of the contract for a period of min. 5 working days min. 6 hours a day, confirmed by personal certificates for all participants,

(d) refresher training for medical staff min. 3 persons at the Ordering Party's headquarters for a period of min. 5 working days, min. 6 hours a day at a time agreed with the Ordering Party, confirmed by personal certificates for all participants,

e) provision of warranty / service services for a period of 24 months, counted from the date of signing the final delivery and acceptance protocol by both parties.
