Investor's supervision over the implementation of the task called "Revitalization and thermo-modernization of public utility buildings intended for cultural and educational purposes

The subject of the contract is the service of investor's supervision over the implementation of the task entitled "Revitalization and thermo-modernization of public utility buildings intended for cultural and educational purposes". The supervision inspector will be responsible for supervising construction works covered by the scope of project documentation and will be responsible for accepting material and installation solutions. The duties of the investor's supervision inspector will include the full range of activities specified in the provisions of the Act of 7 July 1994. Construction Law (Journal of Laws of 2021, 1 item 2351, as amended). The detailed scope of the subject of the contract is specified in: Draft Provisions of the Agreement (PPU) – Annex 8 to the SWZ. and Description of the subject of the contract – Annex 11 to SWZ

The detailed scope of construction works, conditions and method of their execution are specified in the Technical Specification for the Execution and Acceptance of Works (STWOiRB) for individual tasks - Annex No. 6 to the SWZ and design documentation for individual tasks - Annex 7 to the SWZ. Criteria for the evaluation of offers: price-60%, Professional experience of the inspector of the construction and construction industry without restrictions-40%. Economic operators may jointly apply for the award of a contract. In such a case, the Contractors shall appoint a representative to represent them in the procurement procedure or to represent them in the procedure and to conclude a public procurement contract. The power of attorney should be attached to the offer in electronic form. The Contractor may entrust the performance of part of the contract to the Subcontractor.The Contracting Authority does not reserve the obligation for the Contractor to personally perform key parts of the contract. The offer must contain:1)The offer form drawn up according to the model of the Offer Form constituting Annex 1 to the SWZ. In the case of submitting a tender on a form other than Annex 1 to the SWZ, it should contain all the required information specified in this Annex.2)Single European Procurement Document (Annex 2 to the SWZ),3)Obligation of another entity to make available the necessary resources to the Contractor (if applicable),4)Declaration of exclusion, drawn up in accordance with the template constituting Annex 10 to the SWZ, 5) documents from which the right to sign the offer arises (appropriate powers of attorney),6) power of attorney to represent Contractors jointly applying for the award of a contract in a procurement procedure or to represent them in the procedure and conclude a public procurement contract (if applicable). The offer should be prepared in Polish and submitted under pain of nullity in electronic form with a qualified electronic signature, via the Platform available at the address https://umchelm.ezamawiajacy.pl.W the procedure in question, communication between the Ordering Party and the Contractor, including the submission of tenders, exchange of information and transmission of documents or statements, taking into account the exceptions provided for in the Act, takes place using means of communication electronically, via the Platform located at: https://umchelm.ezamawiajacy.pl in the "Correspondence" tab. In accordance with Articles 13 and 14 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Dz. UE L 119 of 04.05.2016, p. 1), hereinafter referred to as "GDPR", the Ordering Party informs that: 1) is the administrator of the Contractor's personal data and persons whose data the Contractor has provided in this proceeding. From 1 January 2022, the Contractor is obliged to ensure at least 10% of the share of electric vehicles or vehicles powered by natural gas in the fleet of motor vehicles within the meaning of Article 2(33) of the Act of 20 June 1997 – Road Traffic Law used in the performance of this task, in accordance with Article 68(3) of the Act of 11 January 2018 on electromobility and alternative fuels (i.e. Journal of Laws of 2022, item 1083, as amended).
