The subject of the contract is the provision of mobile telephony and data transmission services for the National Fund for Environmental Protection and Water Management.

The subject of the contract includes:

1) Provision of mobile telephony and data transmission services (hereinafter referred to as mobile telephony), as described in Chapter I of Annex 5 to the SWZ;

2) Provision of mobile Internet services (mobile links for data transmission), as described in Chapter II of Annex 5 to the SWZ.

A detailed description of the subject of the contract is included in Annexes 4 and 5 to the SWZ.

1. The subject of the contract is the provision of mobile telephony and data transmission services for the National Fund for Environmental Protection and Water Management.

The subject of the contract includes:

1) Provision of mobile telephony and data transmission services (hereinafter referred to as mobile telephony), as described in Chapter I of Annex 5 to the SWZ;

2) Provision of mobile Internet services (mobile links for data transmission), as described in Chapter II of Annex 5 to the SWZ.

2. The following services shall be provided under the basic contract:

2.1. mobile telephony comprising:

a) in the first stage, for numbers held by the Ordering Party, one-time delivery and activation of 481 SIM cards within such a period that the services are available within the time limit assumed by the Ordering Party, i.e. from 01.08.2023, and if the contract is concluded after July 27, delivery and activation of these cards will take place within 5 business days from the date of conclusion of the contract;

b) in the second stage, one-time delivery and activation of 259 new SIM cards within 10 working days of receipt of the Ordering Party's order by the Contractor, whereby the order will be sent to the Contractor not earlier than 01.09.2023 and no later than 01.02.2024;

c) providing the Contracting Authority with other telecommunications services of the Contractor (in accordance with the Contractor's price list for business customers) up to the total amount of PLN 60,000.00 net during the term of the Agreement.

2.2. mobile Internet (mobile connections for data transmission) including one-time delivery and activation of 120 SIM cards within such a period that the services are available within the time limit assumed by the Ordering Party, i.e. from 01.08.2023, and if the contract is concluded after July 27, 2023, delivery and activation of these cards will take place within 5 business days from the date of conclusion of the contract.

A detailed description of the subject of the contract is included in Annexes 4 and 5 to the SWZ.

The term of the contract:

The Contracting Authority requires that the provision of services takes place for a maximum period of 36 months from the date of conclusion of the contract or less, in the event of earlier exhaustion of the amount referred to in §4 section 1 of Annex 4 to the SWZ, whereby:

(a) for a basic contract, the following shall be required:

- in the first stage, for numbers owned by the Ordering Party, the provision of mobile telephony services took place on such a date that the services were available within the period assumed by the Ordering Party, i.e. from 01.08.2023, and if the contract is concluded after July 27, that the delivery and activation of these cards takes place within 5 business days from the date of conclusion of the contract;

- in the second stage, the delivery and activation of new SIM cards took place within 10 working days of receipt by the Contractor of the Ordering Party's order, whereby the provision of mobile telephony services will take place on time, from the activation of SIM cards until the end of the contract;

- provision of mobile Internet services (mobile data connections), including one-time delivery and activation of 120 SIM cards, took place on time so that the services were available within the time limit assumed by the Ordering Party, i.e. from 01.08.2023, and if the contract is concluded after July 27, 2023, that the delivery and activation of these cards takes place within 5 business days from the date of conclusion of the contract.

(b) for contracts under the right of option, it shall be required that:

- the provision of mobile telephony and mobile internet services took place from the date of activation of optional mobile telephony and mobile internet (in accordance with the Contractor's offer) counting from the notification, until the end of the contract period, while the activation period may not be longer than 12 working hours from the notification;

- the provision of data transmission services as part of optional packages, took place from the date of activation of optional data packages (in accordance with the Contractor's offer) counting from the notification, while the activation period may not be longer than 12 working hours from the notification.
