The subject of the contract is the design, supply, assembly and commissioning of the installation of biomass boiler rooms for private residential buildings, the installation of gas boiler rooms for private residential buildings and the installation of gas boiler rooms for public utility buildings in the Municipality of Opoczno, which are implemented as part of the project entitled: "Reduction of pollutant emissions through the replacement of heat sources in the Municipality of Opoczno".

4.2.1.PART 1 OF THE ORDER – Design, delivery, assembly and commissioning of the installation of a biomass boiler room for private residential buildings in the Municipality of Opoczno, which is implemented as part of the project entitled: "Reduction of pollutant emissions through the exchange of heat sources in the Municipality of Opoczno" including in particular:

1) preparation of design documentation (obtaining the decisions of the architectural and construction administration body required by law, including: notification of construction works, building permits, if required and other decisions, permits, arrangements, notifications, certificates required by law),

2) delivery of equipment and installation elements (including the CO detection system), on-site dismantling of existing equipment and installation of boiler installations based on the design documentation along with commissioning,

3) replacement of 15 pieces of biomass boiler room installations located in the Properties as below:

a) set 1: wood gasification boiler with a minimum capacity of 19.5 kW – 1 pc,,

b) set 2: boiler for wood gasification with a minimum capacity of 19.5 kW with a capacitive HOT WATER heater – 2 pcs.,

c) set 3: biomass boiler (pellets) with a minimum capacity of 14.5 kW – 2 pcs.,

d) set 4: biomass boiler (pellets) with a minimum capacity of 14.5 kW with a capacitive HOT Water heater – 4 pcs.,

(e)set 5: biomass boiler (pellets) with a minimum capacity of 19.5 kW – 1 pc,,

f) set 6: biomass boiler (pellet) with a minimum capacity of 19.5 kW with a capacitive HOT Water heater – 5 pcs.,

4) training of Real Estate Residents in the principles of proper operation of the installed boiler along with the development of detailed operating instructions and their transfer to the Residents.

4.3.The detailed scope of work to be performed by the Contractor for all three parts of the contract is described in the PFU, which constitutes Appendix No. 1 to the SWZ.

4.2.2.PART 2 OF THE ORDER - design, delivery, assembly and commissioning of gas boiler rooms for private residential buildings in the Municipality of Opoczno, which is implemented as part of the project entitled: "Reduction of pollutant emissions through the exchange of heat sources in the Municipality of Opoczno" including in particular:

1) preparation of design documentation (obtaining the decisions of the architectural and construction administration body required by law, including: notification of construction works, building permits, if required and other decisions, permits, arrangements, notifications, certificates required by law),

2) delivery of equipment and installation elements (including gas and CO detection system), construction of an external gas installation (to a gas box or LPG tank), delivery and assembly of an LPG tank with a foundation and grounding for liquefied gas boilers, on-site dismantling of existing equipment and installation of boiler installations based on the design documentation along with commissioning.

3) preparation by the Contractor for the user of all documents required for the receipt of the LPG tank by the UDT, and the user will apply for collection to the UDT,

4) replacement of 42 pieces of gas boiler room installations for private residential buildings as follows:

Installations located in a residential building with a usable area of up to 300 m2:

(a)set 1: 1-function natural gas boiler with a capacity of min. 14,5 kW – 3 pcs.

(b)set 2: 1-function natural gas boiler with a capacity of min. 14.5 kW with capacitive HOT WATER heater – 10 pcs.

(c)set 3: 1-function natural gas boiler with a capacity of min. 19,5 kW – 1 pc

(d)set 4: 1-function natural gas boiler with a capacity of min. 19.5 kW with capacitive HOT WATER heater – 8 pcs.

(e)set 5: 1-function natural gas boiler with a capacity of min. 27.5 kW with capacitive HOT WATER heater – 3 pcs.

(f)set 6: 2-function natural gas boiler with a capacity of min. 21,5 kW – 7 pcs.

(g)set 7: 1-function liquefied petroleum gas boiler with a power of min. 14.5 kW with capacitive HOT WATER heater – 2 pcs.

(h)set 8: 1-function liquefied petroleum gas boiler with a power of min. 19.5 kW with capacitive HOT WATER heater – 4 pcs.

(i)set 9: 2-function liquefied petroleum gas boiler with a power of min. 21,5 kW – 2 pcs.

(j)set 10: 2-function boiler for liquefied petroleum gas with a power of min. 25,5 kW – 1 pc

Installations located outside the apartment building:

(a)set 1: 1-function liquefied petroleum gas boiler with a power of min. 19.5 kW with capacitive HOT WATER heater – 1 pc

5) training of Real Estate Residents on the principles of proper operation of the installed boiler along with the development of detailed operating instructions and their transfer to the Residents.

4.3. The detailed scope of work to be performed by the Contractor for all three parts of the contract is described in the PFU, which constitutes Appendix No. 1 to the SWZ.

4.2.3.PART 3 OF THE ORDER - Design, delivery, assembly and commissioning of gas boiler rooms for public utility buildings in the Municipality of Opoczno, which is implemented as part of the project entitled: "Reduction of pollutant emissions through the exchange of heat sources in the Municipality of Opoczno" including in particular:

1) preparation of design documentation (obtaining the decisions of the architectural and construction administration body required by law, including: notification of construction works, building permits, if required and other decisions, permits, arrangements, notifications, certificates required by law),

2) delivery of equipment and installation elements (including gas and CO detection system), construction of an external gas installation (to a gas box or LPG tank), delivery and assembly of an LPG tank with a foundation and grounding for liquefied gas boilers, on-site dismantling of existing equipment and installation of boiler installations based on the design documentation along with commissioning.

3) preparation by the Contractor for the user of all documents required for the receipt of the LPG tank by the UDT, and the user will apply for collection to the UDT,

4) replacement of 7 pieces of gas boiler room installations for public utility buildings as follows:

(a)set 1: 1-function liquefied petroleum gas boiler with a power of min. 27,5 kW – 1 pc

(b)set 2: 1-function liquefied petroleum gas boiler with a power of min. 27.5 kW with capacitive HOT WATER heater – 3 pcs.

(c)set 3: 1-function liquefied petroleum gas boiler with a power of min. 49,5 kW – 1 pc

(d)set 4: 1-function liquefied petroleum gas boiler with a power of min. 49,5 kW with capacitive HOT WATER heater – 1 pc

(e)set 5: 1-function liquefied petroleum gas boiler with a power of min. 99,0 kW with capacitive HOT WATER heater – 1 pc

5) training of people operating the heat source, as to the principles of proper operation of the installed gas boiler along with the development of detailed instructions for use and their transfer to the persons operating the heat source.

4.3.The detailed scope of work to be performed by the Contractor for all three parts of the contract is described in the PFU, which constitutes Appendix No. 1 to the SWZ.
