The subject of the contract is the purchase and successive delivery of dairy products, including m.in milk, butter, cheese, cream, yoghurts for the Fryderyk Chopin University Clinical Hospital in Rzeszów.

A detailed description of the subject of the order and the quantity is specified in the Price and Offer Form (Annex No. 1 to the SWZ), which is an integral part of the SWZ. Information on the rules for the implementation of deliveries is contained in Annex 9 to the SWZ – Draft provisions of the public procurement contract.

The scope of the order includes the delivery of the subject of the order in the quantities specified in the Price and Offer Form (Annex No. 1 to the SWZ), which is an integral part of the SWZ, for a period of 12 months.

The Purchaser does not allow submission of equivalent offers.

Planned delivery of the items of the order. Successive deliveries according to the current needs of the ordering party on an ongoing basis for a period of 12 months from the date of conclusion of the contract. The Ordering Party allows for the possibility of changing the term of the contract in the event of failure to complete the entire subject of the contract during the contract period as a result of the execution of lower orders. An amendment to the contract requires an annex, however, the Contracting Authority unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the term for which it was concluded.
