1. The subject of the order is: successive delivery to the office of the Contracting Authority — pharmacy (including transport and unloading in warehouse) of the pharmaceutical products listed in the price forms annexes 2 to the SWZ.

2. A detailed description of the subject matter of the contract and the conditions of implementation shall

(a) the tender form, which is Annex 1 to the SWZ;

(b) the specific scope of the contract shall be defined by the price forms broken down by sets, which constitute Annex 2 to SWZ;

(c) a draft contract, the model of which is set out in Annex 5 to the SWZ.

3. The place of delivery will be:

(a) Pulmonological and Rheumatological Hospital in Kup, 14 Karola Miarki Street, 46-082 Buy — hospital pharmacy — main building of the Hospital in Buy;

b) Peace Rehabilitation Hospital, 22 Namysłowska Street, 46-034 Room — Pharmacy Department — the main building of the Hospital in Peace.
