The subject of the contract is the conservation of green areas and maintaining cleanliness and order, including conducting a winter action in the city of Płock in Region II.
Cleaning services and maintenance of greenery (mowing lawns, mowing weeds and flower meadows and raking leaves) in the city of Płock in Region II will be provided at the following facilities:
- road lanes of national, provincial, district, municipal and internal streets managed by MZD,
- squares, arcades, playgrounds, gyms, greenery and communal forests,
- other undeveloped areas that are owned by the Municipality and the Treasury.

Cleaning services and maintenance of greenery (mowing lawns, mowing weeds and flower meadows and raking leaves) in the city of Płock in Region II will be provided at the following facilities:
- road lanes of national, provincial, district, municipal and internal streets managed by MZD,
- squares, arcades, playgrounds, gyms, greenery and communal forests,
- other undeveloped areas that are owned by the Municipality and the Treasury.
The boundaries of Region II include the areas lying south of the following streets: Dobrzyńska – Al. Kobylińskiego – Al. Jachowicza together with the area at Stanisławówka – Al. Piłsudskiego together with the road lanes of the above-mentioned streets. The border of the district to the south-east is Wyszogrodzka Street (the section from Spółdzielcza Street to Piłsudskiego Street), South Street (without the road lane of this street) by tracks to the Garrison Cemetery, where the border are posts separating the parking lot from the bicycle path.
The region includes the road lanes of Mostowa Street (section from Kilińskiego Street to the J. Piłsudski Bridge) and the left-bank part of the city, up to the city limits, along with access roads to the Solidarity Bridge from the side of the Radziwie housing estate.
The following are excluded from Division II:
a) areas entrusted by a separate order of ZUM Muniserwis in Płock, e.g.: Region III, Paid Parking Zone, Grodzka Street, Stary Rynek Square,
b) the areas of the Vistula Escarpment,
c) areas and internal roads not owned by the Municipality of Płock or transferred to other organizational units (with the exception of MZD),
square at 42 Popłacńska Street, park square at Jachowicza Avenue, recreation and sports park at Gromadzka Street - only in the field of greenery conservation.
1. The Contractor shall attach to the offer (Offer Form) a statement valid as at the date of submission of tenders confirming the absence of grounds for exclusion and compliance with the conditions for participation in the procedure in the form of the Single European Procurement Document – Annex 1 prepared in accordance with the model standard form specified in the Commission Implementing Regulation (EU) 2016/7 of 5 January 2016. The Ordering Party only allows the alpha section to be filled.
2. The contractor, in the case of reliance on the capacities or situation of the entities providing resources, shall submit, together with the declaration referred to in point 1, also a statement of the provider of resources, confirming the absence of grounds for exclusion of this entity and the fulfilment of the conditions for participation in the procedure accordingly to the extent that the contractor refers to its resources.

1) Power of attorney of the person or persons signing the offer - if the right to sign does not result directly from the attached documents,
2) In the case of submitting a joint offer – power of attorney granted to the leader,
3) Statement of the Contractor jointly applying for the award of the contract submitted pursuant to Article 117 paragraph 4 of the Act – Appendix No. 2,
4) Obligation of the entity providing resources - Appendix No. 3 or other subjective evidence confirming that the Contractor, when performing the contract, will have the necessary resources of these entities /if applicable/,
5) Statement on sanction regulations related to the war in Ukraine – Appendix 4,
6) Proof of payment of the security (if it was submitted in a form other than in cash),
7) Completed Price Form – Annex 5 to the OPZ and to the Agreement.
