The subject of the contract is the management and collection of waste code 19 12 04 – waste generated as a result of segregation from waste collected in a selective manner 15 01 06 mixed packaging waste, coming from Czysta Region Sp. z o. o. in Kędzierzyn-Koźle. The order will be executed successively for 90 days starting from the date of signing the contract up to a total amount of 800 Mg in accordance with the attached schedule specifying the minimum weekly collections.

6.1 The waste covered by this order must be transported and then managed in installations that have decisions allowing the treatment of this waste in a recovery operation at a minimum level of 90 % (an offer with a recovery rate of less than 90 % will be rejected pursuant to Article 226(1)(5) – its content is inconsistent with the terms of the contract, in a manner consistent with applicable law in this respect and the recovery processes specified in Annex 1 to the Act of the on 14 December 2012 on waste (Journal of Laws of 2013, item 21) The Service Contractor should subject the collected waste to fuel production processes 19 12 10 (should have appropriate decisions) or other processing process (e.g. material recovery), which he will confirm with a statement (Paper 21).
6.2 The Customer expects the recovery process to take place in the direction of the production of alternative fuel, if residues are formed, he expects their management to consist in disposal.
6.3 The collection of waste will take place by transport organized by the Contractor and at his expense, also in terms of transport costs, insurance for transport and unloading.
6.4 The daily loading capacity is approximately 200 Mg. These are estimated quantities, used as an auxiliary for the preparation of the offer, with 800 Mg being the maximum amount to be collected and recovered, which the Ordering Party does not have to implement in full. Waste collection will result from the current needs of the Ordering Party.
6.5 Waste generated in a mechanical process is baled and in this form will be loaded onto the means of transport provided. The average weight of 1 m3 of waste code 19 12 04 covered by the contract is about 0.35-0.45 Mg. This is an approximate value used by the Contractor to calculate costs, but it does not constitute the basis for any claims for the amount of work or costs needed to perform the contract. The contractor may take samples of the produced material at his own expense in order to test and determine the morphological composition.
6.6 The service should be provided in the system of successive waste collection, using the Contractor's means of transport on working days between 7.00 and 21.00. The place of waste collection is the area of Czyste Region Sp. z o. o. in Kędzierzyn-Koźle, 7 Naftowa Street.
6.7 Loading will take place with the Ordering Party's equipment, enabling efficient filling of the Contractor's means of transport, e.g. containers, semi-trailers, semi-trailers with a movable floor, etc.
6.8 Weighing of waste collected by the Contractor will take place on a legalized scale, located in Czyste Region Sp. z o. o. The weight determined by the Ordering Party has a weighing accuracy of up to +/- 10 kg. Weighing will be confirmed each time with a weighing receipt. These documents will constitute the sole basis for determining the amount of waste collected between the Ordering Party and the Contractor. After the end of each month, the Contractor will confirm the amount of waste collected in a given month on the basis of the balance of weighings from the computer program operating the weighing system at Czyste Region Sp. z o. o. in Kędzierzyn-Koźle. KPO Waste Transfer Cards will be issued in the BDO system.
6.9 The Contracting Authority shall settle accounts with the Contractor on the basis of a printout from the weighing system and a statement (Form No. 21) and a correctly issued invoice by the Contractor. The unit price per 1 Mg calculated by the Contractor will be the basis for determining the amount of waste actually collected in a given month.
