The subject of the order is the delivery of single-use medical equipment for the needs of the Beskidy Centre for Oncology — John Paul II Municipal Hospital in Bielsko-Biała in the assortment and quantities specified in Annex 5 to SWZ. By delivery, the Ordering Party means the cyclical deliveries of single-use medical equipment during the term of the contract, in accordance with the Ordering Party's orders tailored to his current needs. Deliveries will be carried out as appropriate by the Contractor on the dates set with the Contractor on working days from 8.00 to 12.00. The assortment included in the assortment and price form forming Annex 5 to SIVZ should be delivered according to the needs of the Ordering Party to the Hospital Pharmacy warehouse located in Bielsko-Biała at LiberLiberLiberLiberation Street 18, as well as to the Pharmacy warehouse located in Bielsko-Biała - Biała at 21 Wyspiański Street according to the address indicated in the order.
