1. The subject of the order is group life insurance for employees and family members of employees of Centrum Zdrowia Mazowsza Zachodni Sp. z o. o. in Żyrardów.

2. The Employer does not guarantee that all employed employees and family members of the Employer's employees will join group life insurance.

3. There may be changes in the number of insured persons during the contract period.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay insurance premiums for them.
