Pan Essex CYPCC covering — 7 Clinical Commissioning Groups — Working across 3 sustainability and transformation partnerships — working with 3 local authorities — DOH (2016) National Framework for Children and Young People's Continuing Care. The service aims are the delivery of bespoke packages of continuing care for those children and young people under the age of 18 who have continuing care needs that cannot be met by existing universal and specialist services alone. Contract duration 3 years with the provision to extend for a further 24 months.
