Description:

The subject of the contract is the collection, transport and management of waste generated in the Waste Treatment Plant "Gwarków" OUZ-2 as a result of waste incineration for the Municipal Enterprise of Purification in the capital city of Warsaw sp. Z oo with the division into the tasks specified below:

Task 1: Collection, transport and management of waste with code 19 01 07 * in the planned amount of 1 210 Mg;

Task 2: Collection, transport and management of waste with code 19 01 10 * in the planned quantity of 50 Mg;

Task 3: Collection, transport and management of waste with code 19 01 13 * in the planned quantity of 275 Mg;

Task 4: Collection, transport and management of waste with code 19 01 12 in the planned amount of 17,700 Mg;

In accordance with the requirements set out in the ToR.

Task 1 - Collection, transport and management of waste with code 19 01 07 * in the planned amount of 1 210 Mg

1. Waste collected from the location indicated by the Employer will be directly directed to the Contractor's installation indicated in the submitted bid, where it will be managed in accordance with applicable regulations in this respect.

2. The Purchaser undertakes to:

a) security for the purposes of this waste collection contract 19 01 07 * (task 1) collection will take place from Monday to Friday between from 07:00 to 14:00 from the area of the Waste Treatment Plant "Gwarków" OUZ 2 at ul. Zabraniecka 2 in Warsaw.

b) provide the Contractor's vehicles with entry to the Plant for loading.

c) loading the waste subject to the contract.

3. The contractor is required to have throughout the duration of the contract:

3.1. the current entry in the register referred to in art. 50 paragraph 1 point 5 lit. b of the Act of 14 December 2012 on waste (consolidated text of the Journal of Laws of 2018, item 992 as amended) in the scope of transport of waste covered by the subject of the contract,

3.2. the current decision authorizing waste treatment, including recovery and / or disposal, issued on the basis of the Act of 14 December 2012 on waste (consolidated text of the Journal of Laws of 2018, item 992, as amended) or integrated permit / permit taking into account the processing of waste covered by the contract issued pursuant to the Act of 27 April 2001. Environmental Protection Law (consolidated text, Journal of Laws of 2018, item 799, as amended), including the processing of waste covered by the subject of the contract, with recovery and / or disposal permits in decisions in quantities not less than: for tasks 1 - 1 210 Mg / year.

4. The Employer reserves the right to check, before signing and during the term of the contract, the compliance of the service in the field of waste treatment, including the compliance of the processing process carried out by the Contractor with the documents referred to above.

5. Purchase of BIG-BAG bags 13H3, 90x90x160, made of PP, permanently marked with print in accordance with the requirements of ADR - "Environmentally hazardous material" and "Various hazardous materials and items" on two opposite sides, with the relevant UN code, date of manufacture, information about the attestation and delivering them to the Employer to the address of the Waste Treatment Plant "Gwarków" OUZ 2 at ul. Zabraniecka 2 in Warsaw will take place at the expense of the Contractor. The purchased sacks should meet the requirements of B3 of the European agreement on the international road transport of dangerous goods (ADR) of 30 September 1957 (Journal of Laws of 2015, item 882 as amended) regarding dust and water resistance (applies to task 1).

6. The average monthly amount of waste to be collected is:

Task 1: 19 01 07 * - 80.00 Mg (1m3 of waste with code 19 01 07 * weighs about 0.81 Mg)

7. The contract will be implemented within a period of 12 months from the date of signing, with the possibility of extension for 6 months.

Other requirements, in accordance with the ToR.

Task 2 - Collection, transport and management of waste with code 19 01 10 * in the planned quantity of 50 Mg

1. Waste collected from the location indicated by the Employer will be directly directed to the Contractor's installation indicated in the submitted bid, where it will be managed in accordance with applicable regulations in this respect.

2. The Purchaser undertakes to:

a) security for the purposes of this waste collection contract 19 01 10 * (task 2), collection will take place from Monday to Friday between from 07:00 to 14:00 from the area of the Waste Treatment Plant "Gwarków" OUZ 2 at ul. Zabraniecka 2 in Warsaw.

b) provide the Contractor's vehicles with entry to the Plant for loading.

c) loading the waste subject to the contract.

3. The contractor is required to have throughout the duration of the contract:

3.1. the current entry in the register referred to in art. 50 paragraph 1 point 5 lit. b of the Act of 14 December 2012 on waste (consolidated text of the Journal of Laws of 2018, item 992 as amended) in the scope of transport of waste covered by the subject of the contract,

3.2. the current decision authorizing waste treatment, including recovery and / or disposal, issued on the basis of the Act of 14 December 2012 on waste (consolidated text of the Journal of Laws of 2018, item 992, as amended) or integrated permit / permit taking into account the processing of waste covered by the contract issued pursuant to the Act of 27 April 2001. Environmental Protection Law (consolidated text, Journal of Laws of 2018, item 799, as amended), including the processing of waste covered by the subject of the contract, with recovery and / or disposal permits in decisions in quantities not less than: for task 2 - 50 mg / year;

4. The Employer reserves the right to check, before signing and during the term of the contract, the compliance of the service in the field of waste treatment, including the compliance of the processing process carried out by the Contractor with the documents referred to above.

5. Task 3 - Collection, transport and management of waste with code 19 01 13 * in the planned quantity of 275 Mg

1. Waste collected from the location indicated by the Employer will be directly directed to the Contractor's installation indicated in the submitted bid, where it will be managed in accordance with applicable regulations in this respect.

2. The Purchaser undertakes to:

a) security for the purposes of this contract for waste collection 19 01 13 * (task 3) - collection will take place from Monday to Friday between from 07:00 to 14:00 from the area of the Waste Treatment Plant "Gwarków" OUZ 2 at ul. Zabraniecka 2 in Warsaw.

b) provide the Contractor's vehicles with entry to the Plant for loading.

c) loading the waste subject to the contract.

7. The contract will be implemented within a period of 12 months from the date of signing, with the possibility of extension for 6 months.

Other requirements, in accordance with the ToR.

Task 4 - Collection, transport and management of waste with code 19 01 12 in the planned amount of 17,700 Mg

1. Waste collected from the location indicated by the Employer will be directly directed to the Contractor's installation indicated in the submitted bid, where it will be managed in accordance with applicable regulations in this respect.

2. The Purchaser undertakes to:

a) security for the purposes of this waste collection agreement 19 01 12 (task 4) - collection will take place from Monday to Friday between from 07:00 to 20:00 from the area of the Waste Treatment Plant "Gwarków" OUZ 2 at ul. Zabraniecka 2 in Warsaw.

b) provide the Contractor's vehicles with entry to the Plant for loading.

c) loading the waste subject to the contract.

3. The contractor is required to have throughout the duration of the contract:

3.1. the current entry in the register referred to in art. 50 paragraph 1 point 5 lit. b of the Act of 14 December 2012 on waste (consolidated text of the Journal of Laws of 2018, item 992 as amended) in the scope of transport of waste covered by the subject of the contract,

3.2. the current decision authorizing the processing of waste, including recovery and / or disposal, issued on the basis of the Act of 14 December 2012 on waste (consolidated text, Journal of Laws of 2018, item 992, as amended) or an integrated permit / permit taking into account the processing of waste covered by the contract issued pursuant to the Act of 27 April 2001. Environmental Protection Law (consolidated text, Journal of Laws of 2018, item 799 as amended), including the processing of waste covered by the subject of the contract, with recovery and / or disposal permits in decisions in quantities not less than:

4. The average monthly amount of waste to be collected is:

Task 4: 19 01 12 - 1,200.00 Mg (1m3 of waste 19 01 12 weighs about 1.70 Mg)

5. The Employer reserves the right to check, before signing and during the term of the contract, the compliance of the service in the field of waste treatment, including compliance of the processing process carried out by the Contractor with the documents referred to above.

6. The contract will be implemented within a period of 12 months from the date of signing, with the possibility of extension for 6 months.
