Transport and transport of students with a certificate of the need for special education from the area of Siemianowice Śląskie to / from educational institutions along with the provision of a tutor during the journey

1. The subject of the contract is the transport of students from the area of Siemianowice Śląskie to schools and educational institutions with the provision of a tutor.

2. The order item was divided into 2 parts.

2.1. Part 1 of the contract: "Transport and transport of pupils with a certificate of the need for special education from the area of Siemianowice Śląskie to/from educational institutions together with

with the provision of a guardian during the passage in Siemianowice Śląskie".

2.2. Part 2 of the contract: "Transport and transport of pupils with a certificate of the need for special education from the area of Siemianowice Śląskie to/from educational institutions together with

with the provision of a guardian during travel outside the city of Siemianowice Śląskie".

Transport and drop-off of students with a certificate of the need for special education from the Siem area. Sl. to/from educational institutions with the provision of a guardian during the passage in Siem. Sl.

1.The subject of Part 1 of the contract is the transport and drop-off of pupils with a certificate on the need for special education in accordance with the provisions of the Act of 14 December 2016. Education law to/from educational units in Siemianowice Śląskie with the provision of a guardian during the journey, i.e.:

Route 1:Special Schools Complex in Siemianowice Śląskie, 6 Myśliwiecka Street.

Route 2:Primary School No. 20 with Integration Classes Broniewskiego in Siemianowice Śląskie, Lipowa 3.

2. Detailed description of the subject of the order with respect to the Parts

1 order was included in Appendix No. 3a to the SWZ and in the draft provisions of the contract (Appendix No. 4 to the SWZ).

3. The Contractor is obliged to perform the contract on the terms and conditions described in the draft provisions of the contract (Appendix No. 4 to the SWZ).

4. The Contracting Authority shall not reserve the right to apply for the award of a contract only by Contractors referred to in Article 94 of the PPL, i.e. having the status of a sheltered workshop, social cooperatives and other contractors whose main purpose or main purpose of the activities of their organisationally separated units that will perform the contract is the social and professional integration of socially marginalised persons.

5. In the description of the subject of the contract, the contracting authority took into account the requirements regarding accessibility for the disabled and design for all users referred to in Article 100(1) of the Public Procurement Law.

6. In the description of the subject of the contract, the contracting authority did not specify the performance or functionality requirements referred to in Article 101(1)(1) of the Public Procurement Law.

7. The Contracting Authority shall not request a label from the Contractor and shall not indicate the applicable requirements of a specific label referred to in Article 104 of the Public Procurement Law.

8. The Contracting Authority reserves the right to award contracts referred to in Article 214(1)(7) of the Act.

1) Scope of services to be awarded: the contract will consist in repeating similar services as the basic contracts, i.e. on the transport and drop-off of pupils with a certificate on the need for special education in accordance with the provisions of the Act of 14 December 2016. Law on school education to/from educational institutions.

2) The value of this contract shall not exceed 30% of the value of the basic contract.

(3)A contract may be awarded provided that at least one of the following occurs:

a) exhaustion of the amount referred to in §5 clause 1 of the concluded contract within its term,

b) increase the number of children transported on a given route during the school year, beyond the number of seats in the vehicle,

c) in the event of failure to conclude an agreement regarding the transport and drop-off service of pupils with a certificate on the need for special education from the area of Siemianowice Śląskie to/from educational institutions, together with the provision of a tutor during the journey, as a result of the settlement of an open procedure.

4) The award of the contract will be preceded by conducting a single-source procedure, including negotiations with the Contractor.

Transport and transport of students with a certificate of the need for special education from the area of Siem.Śl. to/from educational institutions along with the provision of a guardian during travel outside the city of Siem.Śl.

1. The subject of Part 2 of the contract is the transport and drop-off of pupils with a certificate on the need for special education in accordance with the provisions of the Act of 14 December 2016. Education law to/from educational units located outside the city of Siemianowice Śląskie together with the provision of a guardian during the journey, i.e.:

Route 1: Polish Association for People with Intellectual Disability Circle in Chorzów Rehabilitation and Education Center in Chorzów, 7 Czysta Street.

Route 2:Dr Maria Trzcińska-Fajfrowska Rehabilitation and Education Centre in Katowice, 8 Gościnna Street; Special Primary School No. 6 in Katowice, 2A Zamkowa Street.

Route 3: John Paul II Special Primary School No. 39 in Katowice, 17 Franciszka Ścigały Street; Primary School No. 55 Special in Katowice, Bishop Herbert Bednorz 13.

Route 4: Special School and Educational Centre for Disabled Children and Youth in Dąbrowa Górnicza, 59 Swobodna Street; Special Educational Centre in Będzin, 7 11 listopada Street.

Route 5: Non-public Special Primary School Noah's Ark in Katowice, 59 Gromadzka Street; Non-public Primary School Wesołe Słoneczka in Katowice, 7 Hetmańska Street; Special Vocational School Complex No. 6 in Katowice, 22 Rolna Street, Technical School Complex

and General Education No. 2 in Katowice, 131 Mikołowska Street.

Route 6: School and Preschool Complex for Deaf and Hard of Hearing Children in Katowice, 17 Michała Grażyńskiego Street.

Route 7: Special School and Educational Center in Ziemicice, 120 Mikulczycka Street.

Route 8: Primary School No. 58 with Maria Dąbrowska Integration Classes in Katowice, al. Bolesława Krzywoustego 11; Jan Twardowski Primary School No. 37 with bilingual classes in Katowice, 17 Józefa Lompy Street; Specialist Non-Public Educational and Rehabilitation Kindergarten in Katowice, Ułańska 5A.

Route 9: Special School and Educational Center for Blind and Visually Impaired Youth, in Chorzów, 22 Hajducka Street.

2. A detailed description of the subject of the contract in relation to Part 2 of the contract is included in Appendix No. 3b to the SWZ and in the draft provisions of the contract (Appendix No. 4 to the SWZ).

3. The Contractor is obliged to perform the contract on the terms and conditions described in the draft provisions of the contract (Appendix No. 4 to the SWZ).
