The subject of the order is the successive delivery of medical gloves to the hospital pharmacy warehouse located in the premises of the District Hospital in Rawicz Sp. z o.o. at Gen. Grota Rowecki 6, 63-900 Rawicz.

Disposable diagnostic glovesThe object of the order is the delivery of disposable diagnostic gloves.

The subject of the contract must have certificates, permits, certificates of registration, certificates required by Polish law, on the basis of which it can be placed on the market and use in health care facilities in the Republic of Poland. Medical devices offered must have CE marking. Each of the items offered should be identifiable by a catalogue number placed in the appropriate place in the offer and assortment form. On the confirmation of the requirements laid down by the Contracting Authority, the Contractor shall be obliged to submit documents proving marketing authorisation and application in accordance with:
(a) The Law of 20 May 2010 on Medical Devices (i.e. Journal of Laws of 2020 item 186 as amended);
(b) Regulation of the Minister of Health of 5 November 2010 on the classification of medical devices (OJ 2010, No 215, item 1416);
(c) Regulation of the Minister of Health of 17 February 2016 on essential requirements and procedures for assessing the conformity of medical devices (OJ 2016, item 211).
Pursuant to Article 13 (1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals in connection with the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Regulation data protection) (OJ EU L119, 4.5.2016, p. 1), hereinafter “GDPR” - thus personal data provided by the contractor will be processed in accordance with the GDPR and in accordance with national regulations. The personal data of the contractor will be processed pursuant to Article 6 (1) (c GDPR for a purpose related to the public contract award procedure No NLO-3820-01/PN/21 conducted in basic mode without negotiation. Recipients of the personal data provided by the contractor will be persons or entities to whom documentation of the procedure in accordance with Article 8 and Article 96 (3) of the Law of Public Procurement (Journal of Laws of 2019, item 2019), as well as Article 6 of the Act of September 6, 2001 on Access to public information. The personal data of the contractor contained in the Protocol of Procedure shall be kept for a period of 4 years from the date of completion of the procurement If the duration of the contract exceeds 4 years, the storage period shall cover the entire duration of the contract. The information clause referred to in Article 13 (1) and (2) of the GDPR shall be found in Chapter III of the Procurement Terms Specification.
