North Staffordshire CCG and Stoke on Trent CCG commission health services for around 492 000 residents and are inviting expressions of interest from providers to deliver a consultant-led multi-disciplinary Community Eye Service to be based in a minimum of 2 sites, one within North Staffordshire and 1 within Stoke on Trent. A number of locally agreed pathways are already commissioned and delivered within the community by Community Optometrists with a Special Interest who have been accredited to deliver these services over a number of years. In addition to these pathways, and because of growing demand within the Local Health Economy for ophthalmological care, commissioners are looking to procure a Community Eye Service that is consultant led and encompasses a multi-disciplinary team approach to ensure that patients are seen in the most appropriate place for their condition, by the most appropriate person and as close to home as possible. The successful provider would be expected to work alongside the existing services as outlined within the service specification and would be required to promote the use of local primary care professionals with the appropriate skills to provide quicker access and choice. The service will be pivotal to the achievement of reduced waiting times by offering more community based services to patients, therefore reducing demand for secondary care and releasing that capacity for those patients with more complex and urgent needs. The service will also benefit patients by delivering a more efficient and effective care pathway. The range of services commissioned would not include the delivery of those services which form part of a NHS general medical or general ophthalmic primary care service contract and will not include services for conditions not outlined within this specification that have been identified conditions that require treatment within an acute tier 4 setting. There is an expectation that the provider will deliver around 4 000 first out patient appointments and 8 000 follow up appointments per annum across both CCGs. North Staffordshire CCG and Stoke on Trent CCG are looking for a provider that can fully meet the specification and demonstrate innovation, best quality, patient experience, cost effectiveness, superior access and deliver seamless, safe patient pathways. There is an expectation that this service will achieve a saving against the existing service model (based on national tariff).
