Delivering the Assessment and review of Eligibility for NHS Continuing Healthcare and Funded Nursing Care for Adults and Continuing Care for Children and Young People across Leicestershire:
1. NHS Continuing Health Care (CHC) and NHS Funded Nursing Care (FNC) for Adults including Personal Health Budgets (PHB).
2. Continuing Care for Children and Young People including PHBs.
3. Health Case Management for Pathway 2; HART and Nursing Home community reablement.
End to end and lifelong service.
The key aims and objectives include meeting the requirements of the following:
— the National Framework for NHS CHC and NHS FNC (revised 2012);
— the National Framework for Children and Young People's Continuing Care (revised 2016);
— National Guidance and regulations regarding the implementation of PHBs including the NHS Direct Payments Regulations (Amended 2013);
— Children and Families Act 2014;
— The SEND Code of Practice January 2015;
— Act as the health case manager for the reablement service for those individuals;
— Assessing patients who have been identified as likely to be eligible for CHC, or who are placed in a nursing home and are eligible for receipt of FNC;
— Reviewing those patients in receipt of fully funded CHC, including fast track funding, joint funded care packages, or FNC;
— Assessing children and young people who have been identified as likely to be eligible for continuing care including those in transition.
