1. The object of the order is to purchase with the transport and delivery of antigen tests for SARS-CoV-2 infection in accordance with the price form.
2. The Contracting Authority informs that it provides for the use of optional orders (option right), in accordance with the provisions of the model contract constituting Annex 7 to the SWZ. In Annex 6 to SWZ (price form), the Contracting Authority shall determine the basic level of the contract, which may extend by increasing the quantity of the sortyment to 50% of the value of the basic contract.
3. A detailed description of the subject matter of the order can be found in the OPZ of Annex 8 to the SIWZ.
4. The item of the order offered may not deviate qualitatively from the desired products listed in column 2 of the “Price Form”. In addition, the Contractor is obliged to provide in column 3 the particulars specifying the product offered, specifying the name of the offered good/manufacturer, a designation which will allow the product to be uniquely identified.
Purchase along with transportation and the introduction of antigen tests for SARS-CoV-2 infection according to the pricing form.
1. The assortment offered should meet the requirements of:
— immunochromatografical (cassette) test detecting the Nucleocapsid antigen of the SARS-CoV-2 virus by sensitivity of not less than 90% and specificity at a level of not less than 97% compared to the RT PCR molecular tests,
— made of biological material — smear from the nasopharynx,
— the characteristics of the test sensitivity and specificity given for the validation data claimed by the manufacturer and based on scientific publications,
— meeting the conditions for use for diagnostic purposes in accordance with the list in the position of the Main Board of the Polish Society of Epidemiologists and Physicians of Infectious Diseases on the diagnostic value of antigen tests used in the diagnosis of SARS-CoV-2 infections,
— wait time for the test result not more than 20 min. ,
— a test registered in Poland as a medical device for in-vitro diagnostics, a POCT test (POCT) test
— the packaging of the test shall contain all the necessary tools and reagents for their performance,
— packaged 25 pcs each in a pooling pack,
— storage conditions: temperature range 2-30
