The subject of the contract are services in the field of voluntary group life insurance for employees and retired prosecutors of the District Prosecutor's Office in Katowice and its subordinate units and their family members, hereinafter referred to as insurance. A detailed description of the subject of the contract is included in Annex 1 to the ToR - Description of the subject of the contract. In the annex the Employer specified the required insurance conditions. The scope and conditions of insurance protection described in the Terms of Reference and annexes are the scope and mandatory conditions, minimal acceptable to the Contractor. The obligatory scope of insurance protection, determined by the type and amount of benefits for five groups of benefits, hereinafter referred to as Variant I, OPTION II, Variant III, Variant IV and Variant V, is set out in Chapter VI of this Annex to the ToR.
