The subject of the contract is a task called: "Transport of students (including the disabled) to schools and educational institutions in the school year 2022/2023" (divided into 9 parts).
Part No. 1: "Providing transport and care during the transport of disabled students from the city of Mysłowice to the institution in Dąbrowa GórniczaPart of the contract No. 1: Provision of transport and care during the transport of disabled students from the city of Mysłowice to the Special School and Educational Center for Disabled Children and Youth in Dąbrowa Górnicza, 59 Swobodna Street, transport route according to Annex 2 to the SWZ – addresses of students transported through the city of Mysłowice (about 80 km there and back). The Ordering Party provides for the transport of up to 12 people with at least one vehicle and the transport of 2 people with one vehicle. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The contractor is responsible for the care and safety of students during transport, on the way from motor vehicles to the facility and from the facility to motor vehicles. The Contractor cooperates with the Contracting Authority, the director of the facility
and parents/legal guardians in the scope covered by the contract. During the entire period of performance of the subject of the contract, the contractor is obliged to provide care for at least two adults to the transported students (guardian – regardless of the driver, at least one guardian in each vehicle). The guardian may be an adult who guarantees safe access of students to and from the institution.
The duties of the guardian include: taking care of the safety of students while driving, helping students getting on and off from motor vehicles, watching over the safe arrival of students to the facility and safe return home. Motor vehicles carrying up to 12 people must be adapted to transport disabled children and disabled youth in wheelchairs. The Ordering Party does not require a vehicle adapted to transport people in wheelchairs in the case of transport for 2 people. The estimated number of days for transport is 190.
Vehicle requirements:
a) They are equipped with inertia seat belts for each passenger and efficient heating and air conditioning of the entire passenger compartment.
b) The vehicle should be marked in accordance with Article 57(1) and Article 58(1) of the Road Traffic Law (Journal of Laws of 2022, item 988).
c) In the event of a vehicle breakdown, the Contractor is obliged to replace a replacement vehicle that meets the requirements specified in the contract within a maximum of 1 hour.
d) The Contractor is obliged to provide vehicles with appropriate tires (summer/winter/multi-season).
e) The Contractor is obliged to ensure that the vehicle is maintained in full technical efficiency and cleanliness, in the winter the vehicle must be heated, ice and snow must not be deposited on the entrance steps.
f) The Contractor should provide a vehicle intended for the transport of disabled people, which has current technical tests allowing the vehicle to be used on the road and mandatory motor insurance of third party liability and accident insurance.
(g) In the case of transport routes involving the carriage of children in wheelchairs, the vehicle must be equipped with an electric lift or ramps, certified, stationary installation of wheelchairs and straps for attaching wheelchairs.
h) The Contractor has telephone or radio communication between the bus driver and the Contractor's dispatching point, which has communication with the police and ambulance.
i) In the case of execution of part of the order No. 1, 3, 4, 5, 7, 8, 9, Vehicles should meet the requirements of Directive 98/69/ EC (exhaust emission standards) – at least the Euro 5 standard.
j) In the case of execution of part of order No. 2, 6, the carriage will be carried out using the vehicle(s) of electromobility, which is understood as an electric or hydrogen-powered vehicle. Bearing in mind Article 68a (1) of the Act on electromobility, it is also allowed to use a hybrid or natural gas vehicle with maximum emissions
Part No. 2: "Providing transport and care during the transport of disabled children and students from the city of Mysłowice to schools and institutions in Katowice".Part of the contract No. 2: Provision of transport and care during the transport of disabled children and students from the city of Mysłowice to the School and Preschool Complex for Deaf and Hard of Hearing Children in Katowice Grażyńskiego 17, Korczakowska Dream School in Katowice, Katowicka 27, Non-Public Primary School "Nasza Szkoła" in Katowice, Harcerzy Września 1939 Street No. 5, Municipal Kindergarten No. 93 in Katowice Łętowskiego 24, Bednorza 13 Street, transport route according to Appendix No. 2 to the SWZ – addresses of children and students transported through the city of Mysłowice (about 130 km there and back). The Ordering Party provides for the transport of up to 15 people by at least one vehicle. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The Contractor is responsible for the care and safety of children and pupils during transport, on the way from motor vehicles to schools, institutions and from schools, institutions to motor vehicles. The Contractor cooperates with the Contracting Authority, directors of institutions and parents/legal guardians in the scope covered by the contract. During the entire period of performance of the subject of the contract, the contractor is obliged to provide care for at least one adult for the transported children and young people (guardian – regardless of the driver, at least one guardian in each vehicle). A guardian may be an adult who guarantees safe access of children and young people to and from schools and institutions. The duties of the guardian include: taking care of the safety of children and students while driving, helping children and students getting on and off from motor vehicles, watching over the safe arrival of children and students to the institutions and safe return home. The estimated number of days for carriage is 253.
Part No. 3: "Providing transport and care during the transport of disabled children and students from the city of Mysłowice to schools and institutions in Mysłowice"Part of the contract No. 3: Provision of transport and care during the transport of disabled children and pupils from the city of Mysłowice to the Henryk Sienkiewicz Primary School No. 13 with Integration and Bilingual Classes in Mysłowice, 14 Hlonda Street, Janusz Korczak Special School Complex in Mysłowice, 4 Wały Street, Integration Kindergarten in Mysłowice, Portowa 11a Street, Kindergarten No. 16 with Integration Classes in Mysłowice, 4 Kawa Street, transport route according to Annex 2 to the SWZ – addresses of children and students transported through the city of Mysłowice (approx. 200 km there and back). The Ordering Party provides for the transport of up to 130 people by at least three vehicles. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The Contractor is responsible for the care and safety of children and pupils during transport, on the way from motor vehicles to schools, institutions and from schools, institutions to motor vehicles. The Contractor cooperates with the Contracting Authority, school and other educational institutions heads and parents/legal guardians in the scope covered by the contract. During the entire period of performance of the subject of the contract, the contractor is obliged to provide care for at least three adults transported children and students (guardians – regardless of the driver, at least one guardian in each vehicle). The guardian may be an adult who guarantees safe access of children and students to and from schools and institutions. The duties of the guardian include: taking care of the safety of children and students while driving, helping children and pupils when getting on and off from motor vehicles, watching over the safe arrival of children and pupils to schools and institutions and safe return home. Motor vehicles must be adapted to transport disabled children and disabled youth in wheelchairs. The estimated number of days for carriage is 253.
Part No. 4: "Providing transport and care during the transport of students (including the disabled) from the city of Mysłowice to the school in Mysłowice (SP 15)".Part of the contract No. 4: Provision of transport and care during the transport of students (including the disabled) from the city of Mysłowice to Primary School No. 15 with Bilingual Classes named after Alfred Szklarski in Mysłowice, 8 Piastów Śląskich Street and 26 Dzierżonia Street, transport route according to Annex 2 to the SWZ – addresses of students transported through the city of Mysłowice (about 30 km there and back). The Ordering Party provides for the transport of up to 55 people by at least one vehicle. The contractor is obliged to equip motor vehicles with means of communication (e.g. a mobile phone), and the vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The contractor is responsible for the care and safety of students during transport, on the way from vehicles to school and from school to vehicles. The Contractor cooperates with the Contracting Authority, the school principal and parents/legal guardians in the scope covered by the contract. The Contractor is obliged, during the entire period of performance of the subject of the contract, to provide care of at least one adult to the transported children (guardian – regardless of the driver, at least one guardian in each vehicle). The guardian may be an adult who guarantees safe access of students to and from school. The duties of the guardian include: taking care of the safety of students while driving, helping students getting on and off vehicles, watching over the safe arrival of students to school and safe return home. The estimated number of days for transport is 190.
Part No. 5: "Providing transport and care during the transport of disabled students from the city of Mysłowice to schools and institutions in Katowice (Giszowiec)".Part of the contract No. 5: Provision of transport and care during the transport of disabled students from the city of Mysłowice to the Dr. Maria Trzcińska-Fajfrowska Rehabilitation and Education Center in Katowice 8 Gościnna Street (Occupational Therapy Workshops - 23 Wojciech Street), transport route according to Annex 2 to the SWZ – addresses of children transported by the city of Mysłowice (about 60 km there and back). The Ordering Party provides for the transport of up to 10 people by at least one vehicle. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The contractor is responsible for the care and safety of students during transport, on the way from motor vehicles to the facility and from the facility to motor vehicles. The Contractor cooperates with the Contracting Authority, directors of institutions and parents/legal guardians in the scope covered by the contract. During the entire period of performance of the subject of the contract, the contractor is obliged to provide care for at least one adult of the transported students (guardian – regardless of the driver, at least one guardian in each vehicle). Part No. 6: "Providing transport and care during the transport of disabled students from the city of Mysłowice to the school in Katowice (Nikiszowiec)".Part of the contract No. 6: Provision of transport and care during the transport of disabled students from the city of Mysłowice to the Special Primary School No. 6 in Katowice, 2A Zamkowa Street, transport route according to Annex 2 to the SWZ – addresses of children transported through the city of Mysłowice (about 60 km there and back). The Ordering Party provides for the transport of up to 10 people by at least one vehicle. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The contractor is responsible for the care and safety of students during transport, on the way from motor vehicles to school and from school to motor vehicles. The Contractor cooperates with the Contracting Authority, the school principal and parents/legal guardians in the scope covered by the contract. Part 7: "Providing transport and care during the transport of disabled students from the city of Mysłowice to the institution in Krakow"Part of the contract No. 7: Provision of transport and care during the transport of disabled students from the city of Mysłowice to the Special School and Educational Center
for the Janusz Korczak Deaf in Krakow, 19 Grochowa Street, transport route according to Appendix No. 2 to the SWZ – addresses of students transported through the city of Mysłowice (approx. 100 km one way). The Ordering Party provides for the transport of up to 4 people. Transport will take place 2 times a week, i.e. delivery on Monday morning, collection on Fridays after classes or other days of the week according to the student's schedule – about 80 trips. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The contractor is responsible for the care and safety of students during transport, on the way from motor vehicles to the facility and from the facility to motor vehicles. The Contractor cooperates with the Contracting Authority, the director of the facility and parents/legal guardians in the scope covered by the contract. During the entire period of performance of the subject of the contract, the contractor is obliged to provide care for at least one adult for the transported students (guardian – regardless of the driver – at least one guardian in each vehicle). The guardian may be an adult who guarantees safe access of students to and from the institution. The duties of the caregiver include: taking care of

Part No. 8: "Providing transport and care during the transport of students (including the disabled) from the city of Mysłowice to the school in Mysłowice (Larysz)"Part of the contract No. 8: Providing transport and care during the transport of students (including the disabled) from the city of Mysłowice (Larysz) to Primary School No. 15 in Mysłowice (Piastów Śląskich Street) – transport to classes at 6:30; 7:30; 8:30; 9:30, pick-up after classes at 13:00; 14:50; 16:30, according to Annex 2 to SWZ. Transport will take place on school days - in accordance with the calendar of the school year - announced annually by the Minister of National Education. The daily route will be approx. 21 km there and back. The Ordering Party provides for the transport of up to 170 people by at least three vehicles. The contractor is obliged to equip motor vehicles with means of communication (e.g. mobile phone), and motor vehicles should meet the technical conditions and have the necessary equipment specified in the Road Traffic Act. The Contractor shall ensure safe and hygienic transport conditions and punctuality of the subject of the contract. The Contractor will be responsible for the care and safety of students (including the disabled) during transport, on the way from motor vehicles to school and from school to motor vehicles. During the entire period of implementation of the subject of the contract, the Contractor will be obliged to provide care for at least three adults transported students (including disabled students, at least one guardian in each vehicle) - guardians - regardless of the driver. The guardian may be an adult who guarantees safe access of students (including the disabled) to and from school. The duties of the guardian will include: taking care of the safety of students (including the disabled) while driving, assistance during getting on and off motor vehicles, watching over safe access to school and safe return to the place designated by the school. The Contractor will cooperate with the Contracting Authority, the school director and legal guardians in the scope covered by the contract. The estimated number of days to be transported is 190
Part No. 9: "Providing transport and care during the transport of disabled students from the city of Mysłowice to the school in Sosnowiec".Part of the contract No. 9: Provision of transport and care during the transport of disabled students (including wheelchair users) from the city of Mysłowice to the Special School Complex No. 4 in Sosnowiec, Franciszkańska 5a, transport route according to Annex 2 to the SWZ - addresses of students transported through the city of Mysłowice (about 30 km there and back). The Ordering Party provides for the transport of up to 5 people with at least one vehicle. The Contractor will be responsible for the care and safety of disabled students during transport, on the way from motor vehicles to school and from school to motor vehicles. The Contractor shall cooperate with the Contracting Authority, the school director and the legal guardian in the scope covered by the contract. During the entire period of implementation of the subject of the contract, the Contractor will be obliged to provide care of at least one adult to the transported disabled students (guardian – regardless of the driver, at least one guardian in each vehicle). The guardian may be an adult who guarantees safe access of disabled students to and from school. The duties of the guardian will include: taking care of the safety of disabled students while driving, assistance during getting on and off motor vehicles, watching over safe access to school and safe return home.
