1. The subject of the contract is the delivery of disposable medical devices used for ablation to the warehouse of the Pharmacy, hereinafter referred to as "products".

2. The type and quantities of disposable equipment are specified in the assortment and price form constituting Annex 1.1 to the SWZ.

3. The required minimum technical and functional parameters of the subject of the contract are described in detail in Annex 6 to the SWZ.

4. The subject of the contract includes:

1) entrusting the Ordering Party with the subject of the order specified in the assortment and price form and its sale in the quantities necessary to supplement the deposit on the terms set out in the draft agreement constituting Annex 2 to the SWZ,

2) leasing to the Ordering Party a system for electrophysiological heart tests and providing instructions in the field of service to persons indicated by the Ordering Party within 3 days from the date of signing the delivery and acceptance protocol, on the terms set out in the draft lease agreement constituting the attachment
