The subject of the contract includes the delivery of medicines to the hospital pharmacy of the Health Care Complex in Pińczów, at 22 Armii Krajowej Street, 28-400 Pińczów. Detailed requirements for the performance of the subject of the contract, its description and quantities are specified in Appendix No. 2 to the Terms of Reference, divided into packages No. 1, 2 and 3.
Package No. 11. The subject of the order includes the delivery of medicines to the hospital pharmacy of the Health Care Team in Pińczów, at 22 Army Krajowa Street, 28-400 Pińczów.
2. Detailed requirements for the implementation of the subject of the contract, its description and quantities are specified in Appendix No. 2 to the Terms of Reference, divided into packages No. 1, 2 and 3.
3. The offered product must be consistent with the description contained in the assortment and price sheet. The Ordering Party allows for the delivery of the product in packages other than those specified in the assortment and price sheet, provided that the total required quantity is properly converted and maintained.
4. Each delivered package of the drug should contain the batch number and expiry date, EAN code, and the expiry date must not be shorter than required for the product.
5. The subject of the contract should meet the relevant conditions for marketing authorisation – medicines should be registered, entered in the Register of Pharmaceutical Products.
6. Names and codes relating to the subject matter of the contract determined in accordance with the Common Procurement Vocabulary.

7. The quantities of individual goods are estimates and may change during the term of the contract depending on the current needs of the Ordering Party, which means that they do not constitute the final dimension of the order, as a result of which they cannot be the basis for filing claims for unfulfilled deliveries or the basis for refusal to perform them.
8. The Ordering Party stipulates that the minimum level of the order will be 80% of the value and quantity of the assortment constituting the subject of the order.
9. The subject of the contract should meet the relevant conditions for admission to trading in the territory of the Republic of Poland.
10. Deliveries will be made successively in accordance with the Ordering Party's demand within a maximum of 4 working days (from Monday to Friday) from placing the order to the fax number indicated by the Contractor, whose offer will be considered the most advantageous. Urgent deliveries will be made in a shorter time after agreement by the Parties. In the case of urgent deliveries, it is allowed to change the assortment, in which case the Ordering Party's approval is required.
11. The goods shall be delivered together with the invoice by the Contractor's own transport or in another way at the Contractor's expense and risk to the Contracting Authority's registered office on working days (from Monday to Friday) from 8 a.m. to 12 p.m.
12. After signing the contract, the Contractor is obliged to deliver with the subject of the contract:
a. leaflets in Polish containing all the necessary information for direct use,
b. instructions in Polish on the possibility of storing and storing the medicine.
13. The Contracting Authority allows the submission of offers with the proposed equivalent drugs, provided that the proposed equivalent drugs have the same spectrum of activity.
14. In the event of withdrawal or discontinuation of production of a given range of drugs and it is not possible to replace it with an equivalent drug with the same spectrum of activity at the stage of preparation and submission of tenders, the Contractor is obliged to: attach an appropriate statement of such fact to the submitted offer, do not value this item, omit it without deleting it. The offer submitted in this way will be valid and will meet the requirements of the Terms of Reference and will be evaluated by the Contracting Authority.
