County Durham and Darlington NHS Foundation Trust is seeking a supplier to provide GP Services to the following Community Hospitals, Sedgefield Community Hospital, Richardson Community Hospital and Weardale Community Hospital. The awarded provider will be required to supply a range of GP Services including but not limited to providing cover from 8:00 — 20:00, 7 days a week. The cover will include 1 ward round every day and the GP will be on-call during the contracted hours if urgent/emergency care or advice is required. The provider will be required to work with a wide range of multi-disciplinary teams and must review prescribed medication and treatment in the patients' health care records and undertake all appropriate admission documentation. The contract will be for 3 years with the option to extend for a further 2 x 12 months. This opportunity is advertised in 3 lots due to the geographical location. Contractors are permitted to apply for 1 lot, 2 lots or all lots. The lots are:

Lot 1- Richardson Community Hospital;

Lot 2- Sedgefield Community Hospital;

Lot 3- Weardale Community Hospital;
