The subject of the contract is to perform inspections, maintenance and maintenance activities of fire protection installations and systems in the objects of the Silesian Museum in Katowice. The subject of the order was divided into two parts: A and B

1. Part A of the contract shall include in particular:

a) carrying out inspections, maintenance and maintenance of the following systems: fire alarm (SAP), audible warning system (DSO), early smoke detection, CO and LPG detection, smoke and fire ventilation and smoke protection, control of FCP control, curtains

smoke and fire gates and fire doors in monthly, quarterly, semi-annual and annual frequencies - according to the Schedule,

b) performing the basic daily and weekly maintenance and maintenance activities described in detail in the OPZ - applies to the fire alarm system (SAP) in Part A of the order and systems in Part B of the order: sprinkler plant, high pressure water mist and gas extinguishing,

c) regular, daily frequency, inspection activities of the hydrant installation and all handheld extinguishing agents, to the extent described in detail in the OPZ,

d) perform 24-hour duty (outside hours on premises), in order to arrive at the facilities covered by this order, in the event of malfunctions in the functioning of the fire-protection systems. Parts A and B of the order and hydrants, within no more than 1 hour from time

notification by telephone or transmitted by electronic means,

(e) coordination and verification of the correctness of inspections, maintenance and maintenance of fire systems contracted in Parts A and B and hydrant installation systems and handheld fire extinguishing agents.

The frequency of carrying out the aforementioned inspections and activities is included in the Schedule constituting Appendix 8A to the SWZ.

Place of execution of the order from Part A:

- SAP and smoke exhalation systems and the aforementioned daily and weekly activities: facilities at 1 T. Dobrowolskiego Street and 3 W. Korfantego Street, Katowice,

- other systems: facilities at 1 T. Dobrowolskiego Street, Katowice.

For a detailed description of the subject of the order, Appendix No. 1A to the In addition, the Contractor is obliged to perform the subject of the contract in accordance with the relevant provisions of the contract - Zał. 2A to the SWZ.

2. Order completion date

The Contractor is obliged to perform the basic contract from the date of conclusion of the contract until 31.12.2022 (hereinafter “basic order”).

3. This order is subject to resumption.

Description of the resume:

1) The Contracting Authority reserves the right to resume this contract, i.e. the obligation of the Contractor to repeat the performance of the object of the contract once between 01.01.2023 and 31.07.2023;

2) The object of the contract under the resumption is the same as the subject of the contract carried out from the date of conclusion of the contract until 31.12.2022, i.e. it will consist in carrying out the same works, activities and maintenance inspections as in the basic order within the scope indicated in

Quotation estimate (Attachments No. 3A to the SWZ);

3) The Contractor shall be obliged to resume the performance of the contract only after the Contracting Authority has provided a statement in this respect, within the deadline by 16.12.2022 at the latest. The statement will be submitted in writing in accordance with the rules of representation of the Contracting Authority;

4) In the event that the Contracting Authority does not submit a statement of resumption in accordance with the rules set out in § 3 (4) of the contract, the Contractor shall not be obliged to perform the resumption of the contract.

4. Pursuant to Article 95 (1) of the PZP Act, the Contracting Authority requires employment by the Contractor or subcontractor on the basis of the employment relationship of persons performing the specified activities in the scope of contract execution. The type of activities related to the requirements of employment of persons on the basis of the employment relationship, the method of verifying the employment of persons, the powers of the Contracting Authority to control the fulfilment of requirements related to the employment of persons by the Contractor and sanctions for non-compliance with these requirements - regulate records of the relevant provisions of the contract - Zał. 2A to the SWZ.

Part B of the order includes inspections, maintenance and maintenance of the following installations: sprinkler, high-pressure water mist, gas extinguishing at monthly, quarterly, semi-annual and annual frequencies - according to the Schedule (Annex 8B to the SWZ).

Place of execution of the order from Part B: objects at 1 T. Dobrowolskiego Street, Katowice.

A detailed description of the subject of the contract is given in Appendix No. 1B to In addition, the Contractor is obliged to perform the subject of the contract, in accordance with the relevant provisions of the contract - Appendix No. 2B to the SWZ
