Provision of support service for 18-25-year-old individuals at risk of homelessness and rough sleeping, including care leavers, to be delivered in purpose-built accommodation, focusing on sustaining living situations, developing independent living skills, engaging in education and training, and achieving independent living within 2 years at Troutbeck Avenue and Longshaw Street, Warrington.
THE PROVISION OF SUPPORT SERVICE FOR SUPPORTED ACCOMODATION FOR 18 – 25-YEAR-OLD SINGLE INDIVIDUALS AT RISK OF HOMELESSNESS, ROUGH SLEEPING AND / OR PREVIOUS CARE LEAVERS NOW AGED 18 +.
Supported accommodation Support Service for 18 – 25-year-olds, residing in the supported accommodation, that have experienced care or have a multiplicity of needs who are at risk, or are at risk of homelessness and rough sleeping, to be delivered in purpose-built accommodation.
Supporting Service Users to, a) sustain their living situation, b) develop and attain their independent living skills, c) engage in learning, education, training, employment, and meaningful activity as individually appropriate to their living arrangements and, d) to achieve a move to independent living, within 2 years. This service is to be delivered from the Supported Housing service at Troutbeck Avenue and Longshaw Street, Warrington.
