1.1. The subject of the contract is the delivery of groceries to the food warehouse of the Wielkopolska Neuropsychiatric Center in Kościan, divided into 18 tasks
1.2. The Contractor is obliged to perform the order on the terms and conditions described in the model contract constituting Annex 7 to the SWZ.
1.3. The Contracting Authority shall allow the possibility of submitting partial tenders for one task, several selected tasks or all tasks.
1.4. The Ordering Party does not allow the possibility of submitting variant offers.
1.5. The Ordering Party does not reserve the obligation of personal performance of deliveries by the Contractor.
1.6. The Ordering Party does not require the Contractor or subcontractor to employ persons performing activities in the scope of the contract on the basis of an employment contract.
Task 1 – Miscellaneous foodstuffs in small packages;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 1, including a description of the assortment and quantity, is Annex 2a.
Task 10 – sauerkraut vegetables;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 10, including the assortment and quantity description, is Annex 2j.
Task 11 – honey and fruit preserves;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 11, including a description of the assortment and quantity, is annexed to Annex 2k.
Task 12 – Miscellaneous foodstuffs in large packages;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 12, including a description of the assortment and quantity, is Annex 2l.
Task 13 – canned vegetables in jars or cans;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 13, including a description of the assortment and quantity, is Annex 2m.
Task 14 – pasta;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party. A detailed description of task 14, including a description of the assortment and quantity, is annexed to Annex 2n.
Task 15 – frozen vegetables and fruits;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week (Monday and Thursday) from 8.00 to 10.00. The Contractor is obliged to deliver a freezer with a minimum capacity of 250 l for the duration of the contract. Any costs of repairing the freezer are covered by the contractor. A detailed description of task 15, including a description of the assortment and quantity, is annexed to Annex 2o.
Exercise 16 — Fish and fish products;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week (Monday and Thursday) from 8.00 to 10.00.
Each batch of delivered goods must have an HDI document. A detailed description of task 16, including the assortment and quantity description, is Annex 2p.
Task 17 – bread and bakery products;The order includes deliveries according to the assortment, appropriate transport of the contractor six times a week from Monday to Saturday from 6.00 to 7.00. A detailed description of task 17, including a description of the assortment and quantity, is Annex 2q.
Exercise 18 – Butter, milk and milk products.The order includes deliveries according to the assortment, delivery by transport of the contractor from Monday to Saturday from 5.00 to 6.00.
Each batch of delivered goods must have an HDI quality certificate. A detailed description of task 18, including a description of the assortment and quantity, is annexed to Annex 2r.
Task 2 – fresh vegetables and fruits;The order includes deliveries according to the assortment, appropriate transport of the contractor five times a week from Monday to Friday until 8.00 to 10.00, along with unloading to the premises of the Ordering Party's food warehouse. A detailed description of task 2, including a description of the assortment and quantity, is annexed to Annex 2b.
Task 3 – fresh meat;The meat supplied must be fresh, not frozen. The surface of the meat must be clean, smooth, untired, unharstened, have a smell characteristic of fresh meat, without signs of beginning to spoilage.
The meat delivered must be veterinaryly tested in accordance with the applicable regulations. The order includes deliveries according to the assortment, specialized transport of the contractor adapted to the transport of food, meeting sanitary and epidemiological requirements, in accordance with applicable regulations.
Delivery of goods twice a week (Tuesday, Thursday) from 8.00 to 10.00.
Delivery of the subject of the order must take place in clean, protecting the goods against damage and contamination packaging with a release to contact with food.
Each batch of delivered goods must have a Commercial Identification Document (HDI) and a quality certificate. A detailed description of task 3, including a description of the assortment and quantity, is annexed to Annex 2c.
Task 4 – fresh poultry;Delivered poultry must be fresh, not frozen. The surface of the poultry must be clean, smooth, unblooded, unharstened, have an odour characteristic of fresh poultry, without signs of beginning to deteriorate.
Delivered poultry must be veterinaryly examined in accordance with the provisions in force. The order includes deliveries according to the assortment, specialized transport of the contractor adapted to the transport of food, meeting sanitary and epidemiological requirements, in accordance with applicable regulations.
Delivery of goods twice a week (Tuesday, Thursday) from 8.00 to 10.00.

The order includes deliveries by assortment. They should take place in a specialized transport of the contractor adapted to the transport of food, meeting sanitary and epidemiological requirements, in accordance with applicable regulations.

Exercise 7 – fresh eggs;The order includes deliveries according to the assortment, appropriate transport of the contractor twice a week from 8.00 to 10.00, on days agreed with the Ordering Party.
