The subject of the contract is a 2-phase innovative approach for diabetes type 2 prevention. The first phase should include the development of systematic approaches for the early diagnosis of people with yet undiagnosed diabetes and for the identification of people at risk. The second phase should consist of the development and evaluation of structured action programmes for diabetes prevention for those at risk. The results of this action should contribute to identify and develop best practices for the prevention of type 2 diabetes. The most successful strategies may be translated or incorporated into future initiatives on diabetes. The impact of the prevention measures developed during the project should be assessed after a meaningful period (at least 2 years) from the beginning of the project. This impact assessment will be done by closely monitoring the behavioural changes of the population participating in the project.
