Execution of construction works related to the implementation of the task entitled: "Expansion of the Hospital Emergency Department in the Radom Specialist Hospital and construction of internal roads".

The subject of the investment is: reconstruction, extension and superstructure of the hospital building, partial reconstruction of the underground communication tunnel, construction of internal roads, squares and parking spaces for the needs of the Hospital Emergency Department of the Dr. Tytus Chałubiński Radom Specialist Hospital on the plot with land no. 3/7 (obr. IX, ark. 90) located at 1 Tochtermana Street in Radom.

6.3. Before selecting the most advantageous offer, the Contracting Authority shall call on the contractor whose offer has been rated the highest to submit within the prescribed period, not shorter than 10 days, the following statements or documents valid as at the date of submission:

6.3.1 List of statements or documents made by the Contractor in the proceedings at the request of the contracting party in order to confirm the absence of grounds for exclusion:

(a) the declaration in the form of a European Single Procurement Document drawn up in accordance with the model standard form set out in Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 at the date of submission. UE nr L 3, 6.1.2016, p. 16/- Annex 2 to SWZ.

In the case of joint application of Contractors for the award of a contract, where the Contracting Authority examines whether there are grounds for exclusion in respect of each of these Contractors, a uniform document shall be submitted by each of the Contractors acting jointly.

b) the Contractor's statement on the validity of the information contained in the statement referred to in Article 125(1) of the PPL Act (ESP), with respect to the grounds for exclusion from the proceedings indicated by the Contracting Authority, referred to in:

a) Article 108 (1) (3) of the Act,

b) Article 108(1)(4) of the Act, concerning the ruling of a final ban on applying for a public contract as a precautionary measure,

(c) Article 108 (1) (5) of the Act concerning the conclusion of an agreement with other contractors to distort competition,

d) Article 108 (1) (6) of the Act,

e) Article 109(1)(8) of the Act

in accordance with Annex 9 to the SWZ.

c) the Contractor's statement, within the scope of Article 108(1)(5) of the Act, on the lack of belonging to the same capital group within the meaning of the Act of 16 February 2007 on competition and consumer protection (Journal of Laws of 2021, item 275), with another Contractor who submitted a separate offer, or a statement on belonging to the same capital group together with documents or information confirming the preparation of the offer, independently of another Contractor belonging to the same capital group, in accordance with Appendix 9 to the SWZ.

d) Statements of the contractor/contractor jointly applying for the award of the contract REGARDING THE GROUNDS FOR EXCLUSION UNDER ARTICLE 5K OF REGULATION 833/2014, in accordance with Annex 5 to the SWZ.

e) information from the National Criminal Register concerning:

Article 108(1)(1) and (2) of the PPL Act, Article 108(1)(4) of the Act, concerning the ruling of a final ban on applying for a public contract - drawn up not earlier than 6 months before its submission

f) a copy or information from the National Court Register or from the Central Register and Information on the Department. Gosp., drawn up not earlier than 3 months before its submission, if separate provisions require entry in the register or records, in order to confirm the absence of grounds for exclusion pursuant to Article 109 paragraph 1 point 4) of the PPL;

5.6. In accordance with Article 139 of the PPL, the contracting authority in the open tender procedure will first examine and evaluate the bids, first examine and evaluate the bids, and then examine whether the contractor whose bid was assessed as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.

8.1. The Contracting Authority requires contractors to pay a security in the amount of: PLN 600,000.00.

8.2. The bid bond must be lodged by 12.09.2023 at 10:00.
