The subject of the contract is group life insurance for employees and family members of employees of the Independent Public Health Care Team

in Myszków.

The Ordering Party does not guarantee that all employed employees and family members of the Ordering Party's employees will join the group life insurance.

During the insurance period, there may be changes in the number of insured persons.

If the employee or a member of the employee's family resigns from insurance, the Ordering Party is not obliged to pay an insurance premium for them.

Detailed terms of the contract are included in Annex 1 to the SWZ.

The professional and age structure of the Ordering Party's employees is presented in Appendix No. 2 to the SWZ.
