Organization of transport of children to schools and kindergartens in the school year 2023/2024.Transport will be provided on the basis of monthly tickets, on lines created by the Contractor, with priority for children and youth with a personal school monthly ticket and carers with a monthly ticket (the price of tickets will be determined in this procedure). In the proceedings, a bid bond of PLN 15,000.00 was established. Before selecting the most advantageous bid, the contracting authority shall call upon the contractor whose bid will be rated the highest to submit within the prescribed period, not shorter than 10 days, valid as at the date of submission of all subjective evidence listed in the SWZ.

They must concern, respectively: the Contractor and all consortium members:1. Extract or information from the National Court Register or from the Central Register and Information on Economic Activity, within the scope of Article 109 paragraph 1 point 4 of the Act, prepared not earlier than 3 months ( ...) c.d. in II.2.11

The subject of the contract are services consisting in the transport of school children and youth from the Góra commune together with their guardians to educational institutions located in the Góra commune and the Wąsosz commune* (*transport of disabled children to the Special School and Educational Center in Wąsosz) along with transport after school classes to their places of residence. Transport will be provided in the school year 2023/2024, in the period from 04.09.2023 to 21.06.2024. The forecasted total number of students commuting monthly to educational institutions is: 331, the estimated number of tutors: 9. Detailed description in SWZ. In accordance with the disposition in art. 95(1) of the PPL, the Contracting Authority describes the obligation of the contractor or subcontractor to employ on the basis of an employment contract persons performing activities related to the transport of passengers, i.e. drivers, if the performance of these activities consists in performing work in the manner specified in Article 22 § 1 – Labour Code. Employment under an employment contract of the above-mentioned persons should continue uninterruptedly throughout the duration of the contract. Statements and documents that the Contractor should submit together with the offer:1) completed offer form together with the completed attachment to the SWZ – calculation of the offer price. The offer must be signed, using a qualified electronic signature, issued by a qualified trust service provider, which is an entity providing certification services - electronic signature, meeting the security requirements set out in the Act of 5 September 2016 on trust services and electronic identification, by persons authorized to represent the Contractor (Contractors jointly applying for the award of the contract),

in accordance with the form of representation of the Contractor specified in the court register or other document appropriate for a given organizational form of the Contractor, or by

authorised representative of the Contractor jointly applying for the award of a contract Article 58(1) – (...) Contractors shall appoint a proxy to represent them in the procurement procedure or to represent them in the procedure and conclude a public procurement contract.1.1. In the case of transmission of an electronic document in a format that compresses the data in proceedings or competitions, affixing a qualified electronic signature to a file containing compressed documents is tantamount to affixing a qualified electronic signature to all documents contained in that file. The contracting authority applies the so-called reverse procedure referred to in Article 139 of the PPL Act, therefore the Contractors do not submit the ESPD together with the bid. 2) A copy or information from the National Court Register or from the Central Register and Information on Economic Activity or other relevant register in order to confirm that the person acting on behalf of the contractor is authorized to represent him, the Contractor shall not attach these documents if he has entered in the offer form the address / s of the free and publicly available database in which they are available and the Contracting Authority may download them himself / This requirement applies to the Contractor, contractors acting jointly and the entity providing resources.

3) power of attorney to sign the contractor's bid, if a person acts on behalf of the contractor, whose authorization does not result from copies or information from the National Court Register CEIDG or other relevant register. The principles of personal data processing are described in the SWZ.
