Reception and management of waste code 19 12 12 from the Waste Management Department

The subject of the contract is the collection and management of waste with the code 19 12 12 from the Waste Management Plant

Reception, transport and management of waste code 19 12 12 from a stream of unsegregated (mixed) municipal waste code 20 03 01

1. The subject of the contract is the provision of management services in the recovery process (in accordance with annex 1 to the Act of 14 December 2012 on waste) by the Waste Contractor code 19 12 12 12, together with the service of their successive reception and transport from the Olszów Waste Management Plant Sp. z o.o. located at: Olszowa, Bursztynowa Street 55, 63-600 Kępno, to the place of their development.

2. The service should be carried out in accordance with decisions held by the Contractor and with any universally applicable law, including in particular:

a) The Waste Act of 14.12.2012. (Journal of Laws of 2022, item 699, as amended);

b) the Environmental Protection Law Act of 27 April 2001 (Journal of Laws of 2022, items 2556, 2687);

3. Waste covered by this order shall be transported and then developed in installations indicated by the Contractor which:

(a) have important decisions for conducting waste recovery activities set out in Annex 1 to the Act of 14 December 2012 on waste code 19 12 12 12 12;

(b) have the required decisions/authorisations/entries in the relevant registers authorising the transport of waste of code 19 12 12 12;

(c) they have an active account in the electronic BDO system, enabling the necessary activities related to waste records (confirmation of the KPO).

4. The Contractor shall provide the Contracting entity, no later than 05.01.2024 and within 10 days from the date of receipt of the last batch of waste, in writing and by e-mail:

a) information confirming the type of recovery process of accepted waste in accordance with Annex 1 of the Waste Act of 14 December 2012;

(b) the quantities of products or waste produced therefrom, indicating the final installation to which they were delivered (name, BDO number with MPD number) together with the final process to which they were subjected.

5. For each call of the Contracting Authority, the Contractor is obliged to provide within 7 days of the call — a summary of data containing the quantities of waste received and processed with information to which processes it has been subjected.

6. The subject of the contract is waste with the code 19 12 12 - ballast after sorting waste with the code 20 03 01 from the fraction above 80 mm

7. Waste will be prepared by the Ordering Party for collection in loose form, bulk density of the fraction in question less than 200 kg/m3.

8. Waste will be stored in a covered box.

Collection, transport and management of waste with code 19 12 12 from the stream of separately collected packaging waste (from waste codes: 15 01 02 and 15 01 06)

1. 6. The object of the order is waste code 19 12 12 - ballast after sorting waste with code 15 01 02 and 15 01 06.

7. Waste will be prepared by the Ordering Party for collection in an unfragmented, pressed form, bound with steel wire.

8. The dimensions of bales of compressed waste are approximately: 80 x 80 x 120 cm, and the average weight of one bale is approx. 450 kg
