Bromley CCG is seeking to appoint an organisation for the provision of a service delivering community cardiac rehabilitation and exercise referral programmes.
The community cardiac rehabilitation programme is to support a hospital based phase 3 and deliver phase 4 of an accredited BACPR approved cardiac rehabilitation programme across Bromley. This programme is for patients who have either had a recent cardiac event or require ongoing cardiac support for an identified heart condition by primary care. Included in this contract will be an exercise referral programme to support patients with long term conditions. This will be delivered in the areas of Orpington, Beckenham and Penge.

The service is to support hospital based phase 3 and deliver phase 4 of an accredited BACPR cardiac rehab programme. The programme is for patients who have had a recent cardiac event or require cardiac support for an identified heart condition. The service will include an exercise referral programme to support patients with long term conditions. This will be delivered in the areas of Orpington, Beckenham and Penge.
The CCG is looking to provide these services from spring/summer 2019, for 3 years with an option to extend for 2 years. The service will operate over a 7 day period between the core hours 8:00 – 20:00 with an out of hours service up until 10:00 p.m. on weekdays. This will be for all adults registered with Bromley GPs, with referrals only by the agreement of a physician operating in primary, secondary or tertiary care.
