The subject of the order is the supply of reagents, controls, calibrators and consumables along with the lease of analyzers and equipment necessary to perform the analyzes, in the assortment and quantities specified in Annexes 2.1 - 2.8 to the ToR - Assortment and quantity form.

The awarding entity allows submitting bids for individual parts of the contract, called packages. This order consists of 8 packages. An offer may be submitted for one or more packages. Packages 1 and 4 consist of several tasks, the offer of the Contractor for a given package must include all tasks contained in it.

The offered object of the contract must be admitted to trading and use as a medical device (only for medical devices), free from any physical or legal defects.

Package 1.

Supply of reagents necessary for the determination of coagulological, morphological and biochemical parameters together with the lease of analyzers

Task 1. Supply of reagents necessary to perform 25 400 determinations of coagulological parameters together with the analyzer lease.

Task 2. Supply of reagents for 90,000 morphology determinations with the lease of two 5 DIFF automatic hematology analyzers, one of the analyzers separating white blood cells into 6 subpopulations.

Task 3. Supply of reagents, controls, system fluids and wearing parts along with the lease of 2 analyzers for 378 350 biochemical tests.

Supply of test strips necessary to perform 22,000 urine analyzes with the lease of a urine analyzer.

Supply of reagents, calibrators, controls and other consumables for performing immunochemical tests for the Cobas analyzer, included in the package: lease of the Cobas e411 supporting analyzer

Supply of reagents, calibrators, controls and consumables necessary to perform tests in the field of allergic disease diagnostics, diagnosis of Lyme disease and celiac disease together with lease, microplate reader, automatic microplate washer, computer program for interpretation of results, scanner for reading test strips.

Task 1. ELISA kits for the determination of antibodies against Borrelia in the IgM and IgG class and confirmation tests for Borrelia IgM and IgG - strip tests - together with the necessary equipment and readers.

Zad 2. Tests for determining allergic profiles with the lease of the necessary equipment for the determination and diagnosis of celiac disease
