1. The subject of this order is the successive supply of consumables for OKI and XEROX printing devices for the Maria Skłodowska-Curie National Institute of Oncology - National Research Institute, Gliwice Branch, hereinafter referred to as the Contracting Authority.

3. Information clause arising from the contents of Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals in connection with the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, The Ordering Authority posted in SWZ.
1. The subject of this order is the successive supply of consumables for OKI and XEROX printing devices for the Maria Skłodowska-Curie National Institute of Oncology - National Research Institute, Gliwice Branch, hereinafter referred to as the Contracting Authority.
2. The description of the subject of the contract and the conditions for the execution of the order are
1) Assortment and price specification – Annex 2 to the SWZ,
2) The designed provisions of the contract to be entered into the contents of the contract — Annex 4 to the SWZ.
3. Each Contractor must be accompanied by the means of evidence in question:
a) A statement by the manufacturers of materials that the offered consumables will come from the distribution channel to the Polish market.
- the evidence referred to in point (a) above shall be supplemented if it is not submitted with the tender or is incomplete,
4. In the case of offering equivalent materials, the Contractor shall attach to the offer the evidence in question, described in detail in the Terms of Reference in Chapter IV and in Chapter VII from point 6 a) to point 6 h).
-The evidence in question concerning equivalence, i.e. those referred to in point 4 above, shall not be supplemented if they are not submitted together with the offer or are incomplete.
5 The Contracting Authority does not envisage concluding a framework agreement, establishing a dynamic purchasing system, electronic auction or supplementary orders.
6. The proceeding does not cover the objections set out in Article 94 of the Pzp Act.
