The object of the order is the renovation and adaptation of the premises for PET-CT, in “design and build” mode.

The contract and the PFU concerns the adaptation of premises in Building B for PET CT and Building Z for the purposes of the Isotope Studio and the execution of full design documentation for the above scope of works, obtaining the necessary permits (Sanepid and State Atomistics Agency), and carry out receipts and transfers for use. The aim of the action is to design and create rooms in Building B for PET CT and rooms in Building Z for the Isotope Studio.

As part of the execution of the order, the necessary design documentation should be developed, in accordance with the applicable regulations and requirements for the reception of premises by the Authorities of the State Administration, including, among others, Sanepid and the State Atomistics Agency — including permits for the use of the apparatus and atelier.

The task is to be completed as comprehensive and complete, communicated to the Contracting Authority, together with the necessary receipts for use without incurring additional costs by the Contracting Authority. Equipment of most rooms is performed as comprehensive and complete with furnishings, equipment, fittings, etc. The undertaking “Adaptation of rooms in building B for the purposes of PET CT and building Z for the purposes of the Isotope Studio in the Institute “Monument — Center for Child Health” — in mode design and build” is planned to be performed as the following tasks:

— Task _1 — the execution of complete design documentation in each of the industries and the Plant Radiological Protection Plan,

— task_2 — adaptation of building premises B and Z — construction work was renovated. Execution of post-construction documentation and obtaining all necessary permits and carrying out receipts and transfer for use.

Task 2 should be done in step. Namely, in the first place, it is necessary to make an adaptation of the premises in the bud. Z, so as to derive the existing laboratory from the part of the area covered by the adaptation of the premises for the needs of PET-CT in the bud. B. This does not mean that in bud. B cannot be carried out at the same time, but outside the above area until the laboratory is transferred to the renovated building. Z. Details to be determined with the ordering authority.

All the robots, will be done:

1) concerning the preparation of the room and installation for the installation of the PET CT device (Positron Emission Tomograph) up to 29.10.2021;

2) remaining until 17.12.2021.

The Contractor whose offer is considered to be the most advantageous shall be obliged to provide a security of due performance of the contract amounting to 5% of the gross value of the contract.
