1. The subject of the order is group life insurance for employees and family members of employees of the Zamość Private Hospital sp. Z o. O. In Zamość.

2. The Employer does not guarantee that all employed employees and family members of the Employer's employees will join group life insurance.

3. There may be changes in the number of insured persons during the contract period.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay an insurance premium for them.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay an insurance premium for them.

Option right:

1. The Awarding Entity provides for the option to exercise the option right by automatically extending the contract for a period of 12 months on the same terms as previously concluded contracts.

2. The Contractor may declare in writing, at least 3 months before the end of the one-year insurance period, that he does not agree to an extension for the next period if the loss ratio is higher than 80%. If the Contractor uses the declaration of disagreement for an extension for the next period, the Contractor is obliged to submit a statement along with a report on the claims.

3. Lack of submission by the Contractor of the statement to the Employer, within the above-mentioned deadline, means an automatic extension of the contract for another 12 months, in this case the contract will last a maximum of 30.6.2023.

4. The statement referred to in para. 2 The Contractor shall submit to the Employer in writing under pain of nullity.
