1. The object of the order is the delivery to the Provincial Hospital in Bielsko-Biała at Al. Armia Krajowa 101, neurostimulators for stimulation of the spinal cord and sacral nerves, according to the quantitative and generic summary set out in Annex No 2 to the Specification of the Order Conditions, called hereinafter “swz”, meeting the qualitative parameters set out in Annex No 2 to swz, under the conditions laid down in the proposed provisions of the contract constituting Annex No 4 to the swz.

2. The offered goods must be new and unused.

3. Delivery of the subject of the order together with transport and unloading will take place at the cost and risk of the Contractor.
