1. The subject of the contract is: SUPPLY OF REAGENTS, CONTROL AND CONSUMABLE MATERIALS IN THE FIELD OF CLINICAL BIOCHEMISTRY AND HAEMOSTASIS TOGETHER WITH THE LEASE OF ANALYSERS for the Provincial Specialist Hospital of the Blessed Virgin Mary in Częstochowa.
2. The description of the subject matter of the order is included in the assortment and price forms forming Annex 2 to the SWZ. The remaining conditions for the fulfillment of the contract are set out in Annex 6 to the SWZ, a model contract.
3. The subject of the order includes 2 parts.
4. The Contracting Authority authorises the submission of partial tenders. The term partial offer shall mean the individual parts set out in Annex No 2 to the SWZ. The Contractor may submit a bid for any parts or for the entire subject of the contract.
