Description:
Collection, transport and disposal of expired medicines from the premises of the Municipal Association of Municipalities of the Leszno Region

The subject of the contract is a comprehensive service of collection, transport and disposal of expired medicines from households, collected in containers set up in points (pharmacies) located in the Municipal Association of Municipalities of the Leszno Region.

The Contractor is obliged to collect and transport expired medicines and ensure their disposal.

The service of collection and management of waste from pharmacies will include:

(a) on-site weighing of waste at the collection point;

(b) the collection of waste from collection points;

(c) loading of waste;

(d) the transport of waste to the place of disposal;

(e) waste disposal.

Currently, there are 84 collection points for expired drugs in the KZGRL. The Ordering Party reserves the right to increase or decrease the number of points collecting expired medicines during the implementation of the task.

An up-to-date list of collection points for expired medicines will be attached to the contract.

The contractor is obliged to equip all collection points (pharmacies on the list) with containers for expired medicines. In the event of the creation of a new pharmacy expressing the desire to collect expired medicines from households, the Contractor is obliged to deliver such a container to the pharmacy indicated by the Ordering Party within 30 days from the date of notification. The Contractor shall notify the Ordering Party about the liquidation of the pharmacy or about another situation in which the collection of expired medicines was not possible. The cost of equipping collection points with the above-mentioned containers should be calculated in the price of the offer.

The Contractor is obliged to collect expired medicines with the frequency of:

- minimum 1 time per month from the city of Leszno and the municipalities of Gostyń and Rawicz,

- a minimum of 1 time every two months from other municipalities.

In the case of faster filling of containers in a given pharmacy, after receiving the appropriate notification from the pharmacy or KZGRL, waste collection should be carried out within 14 days from the date of notification.

The Contractor is obliged to keep records of the acceptance of expired medicines, taking into account:

(1) the date of receipt;

2) names and addresses of meeting points - divided into municipalities;

3) the number of collections made from a given point together with confirmation of the receipt of medicines by an employee of the collection point (stamp and signature);

4) the weight (in kg) of each waste collected from a given point.

The data from the above-mentioned records will be the basis for issuing an invoice for the implementation of the service in the past month.

To issue an invoice, you must accept the weight of medicines collected from the collection points from which the collection actually took place during the past month.

If the Contractor was at the pharmacy point, but did not collect the medicines, due to their absence, a pharmacy stamp confirming the Contractor's visit to this point is required.

The originals of the created records should be delivered to the Ordering Party together with the invoice issued.
