Description:
Purchase and delivery of a PET/CT device for SP ZOZ Municipal Hospital Complex

The subject of this order is the purchase and delivery of a PET/CT device along with the adaptation of rooms for the Department of Radiology, Diagnostic Imaging and Nuclear Medicine located at 7 Władysława Truchana Street. The subject of the contract has been described in detail in tabular form in Annex 2 to the SWZ - Description of the subject of the contract (hereinafter referred to as: OPZ), i.e. parameters, technical requirements, limit and desired conditions.

The subject of this order is the purchase and delivery of a PET/CT device along with the adaptation of rooms for the Department of Radiology, Diagnostic Imaging and Nuclear Medicine located at 7 Władysława Truchana Street. The subject of the contract has been described in detail in tabular form in Annex 2 to the SWZ - Description of the subject of the contract (hereinafter referred to as: OPZ), i.e. parameters, technical requirements, limit and desired conditions.

1.1. ON-SITE VERIFICATION

1. As part of the procedure pursuant to Article 131(2) of the UPZP, the Contracting Authority requires the Contractors to undergo a site inspection at the Department of Radiology, Diagnostic Imaging and Nuclear Medicine, located at Władysława Truchana 7, 41-500 Chorzów (PET/CT Laboratory). The contracting authority sets a condition for the on-site inspection due to the need to perform the contract within a rigidly defined time frame resulting from the grant agreement. Failure to meet the deadlines is associated with the risk of losing funds for the implementation of the subject of the contract. The on-site visit aims, among other things, to:

- identification of the possibility of the entry path of the PET/CT device, including the need to perform disassembly and other works necessary for the safe introduction of the PET/CT device,

- familiarization with the technical infrastructure, including the possibilities of electrical supply, cooling system and other important for the proper installation, foundation, commissioning of the PET / CT apparatus,

- planning the disassembly of the existing PET / CT device, performing adaptation works and installing and commissioning a new device - work in this area must take place immediately one after the other,

- determination of other essential elements necessary for the correct valuation, technical preparation of delivery, disassembly (of the PET/CT machine owned), installation of a new PET/CT machine and other factors relevant to the proceedings.

2. The Contracting Authority shall designate the possibility of a site inspection from 10.07.2023 to 21.07.2023 between 8:00 and 14:00 (excluding public holidays (Sundays and holidays) and Saturdays). The Contractor wishing to carry out a site inspection should notify the Contracting Authority by phone one day in advance. The person authorized to contact regarding the on-site inspection is the Head of the Medical Equipment Department at the telephone number 721-806-533.

3. In order to confirm the fulfillment of the condition of participation in the on-site inspection, the Contractor is obliged to submit together with the offer a statement approved by the Contracting Authority on the completion of the on-site inspection, the model statement constitutes Annex 10 to the SWZ. During the on-site inspection, a memo will be written by the representative of the Contracting Authority, in which the data of the participants and the date of the on-site inspection will be recorded. The Contracting Authority informs that it will not provide any explanations regarding the subject of the contract during the on-site inspection. In the case of Contractors acting jointly, it is required that at least one of the Contractors participate in the vision.

4. The Contractor's offer submitted without a site inspection at the Department of Radiology, Diagnostic Imaging and Nuclear Medicine located at Władysława Truchana 7, 41-500 Chorzów (PET/CT Laboratory) will be subject to rejection under Article 226(1)(18) of the Public Procurement Law.

7.1. By joining the tender, the Contractor is obliged to pay a security in the amount of: PLN 397,000.00.
