1. The subject of this contract is: Delivery of medical equipment to specialist clinics I for the Health Care Team, Baczyńskiego 1, 55-200 Oława.
2. The scope of the contract shall cover 1 lot, including:
Delivery of digital X-ray apparatus
3. Quantities, characteristics and technical and functional parameters of the subject of the contract are specified in the description of the subject of the contract – Annex 2-1 (respectively for individual tasks) and Annexes No. 1, 5 to the specification of the terms of the contract (SWZ).
4. The subject of the contract shall comply with the requirements laid down by the Contracting Authority in:
a) Act on Medical Devices of 7.04.2022. (consolidated text, Journal of Laws of 2022, item 974, as amended),
b) Regulation of the Minister of Health of 17 February 2016 on essential requirements and procedures for conformity assessment of medical devices (Dz.U. of 2016, item 211 with later. Deaths) ;
c) it should meet the technical requirements contained in the SWZ and annexes to the SWZ,

For this purpose, the Contractors shall submit:
- a statement that the offered subject of the contract has valid documents allowing for circulation and use issued in accordance with the law - Appendix No. 8.
5. Wherever the Contracting Authority allows the possibility of submitting equivalent tenders, the description of the subject of the contract specifies the features of equivalence (if applicable). By the term equivalent offer, the Ordering Party shall understand the subject of the contract at least equal in terms of the described norms, standards, parameters, technical and quality features. The described parameters of the subject of the contract constitute the minimum quality required by the Ordering Party. The Contractor who submits an equivalent bid is obliged to inform the Contracting entity about this fact and demonstrate that the subject of the contract offered by it meets the requirements specified by the Contracting Entity, by providing information on the fulfillment of the characteristics indicated by the Contracting Authority as equivalent. Proving equivalence lies with the Contractor.
